云南、四川两省吸毒者吸毒特征及其与HIV感染高危因素的研究
EPIDEMIOLOGICAL STUDY ON THE HIGH RISK FACTORS OF HIV/AIDS AMONG DRUG ABUSERS IN SICHUAN AND YUNNAN PROVINCES
【摘要】 目的 :调查川、滇两省吸毒者吸毒行为和性行为特征及其与HIV感染传播的关系。方法 :自行设计调查问卷 ,在川、滇两省的 11个地、市、县药物依赖治疗机构调查接受治疗的吸毒者。调查内容包括 :⑴调查对象人口学特征 ;⑵药物滥用史 ;⑶注射方式滥用毒品情况 ;⑷性行为。调查除采用结构性访谈填写调查表外 ,对个别问题采用开放式访谈。结果 :在 16 80例有效被调查者中 ,女性占 2 2 3% ;少数民族占 2 4 1% ;高中及高中以上文化占2 5 1% ;职业基本覆盖社会各行业 ,其中无业者和农民分别占 4 1 9%和 2 3 4 % ;离婚、分居和 (未婚 )同居者分别占13 7%、1 5 %和 4 5 %。该群体主要滥用 (成瘾 )的毒品是海洛因 (占 98 4 % )。 1113例在吸毒的某个阶段曾经采用注射方式吸毒 ,占被调查总数的 6 6 3% ;6 39例有共用注射器行为或经历 ,共用注射器的原因主要是“毒瘾上来 ,顾不上别的”(占 5 0 2 % )。该群体在过去一生中有 10 91例 (6 4 9% )有过非婚性活动 ,在过去一年中有 70 9例 (42 2 % )有过非婚性活动 ;性活动中有近一半的人“从来不戴”安全套。在 916例应答者中感染非淋菌性尿道炎、淋病和尖锐湿疣等性传播疾病的分别占 34 8%、2 8 7%和 11 6 %。在 4 0 7例接受过HIV检测者中 ,本人已知HIV阳性 6 2例
【Abstract】 Objective: To survey the relationship between the behavior of drug abuse and infection/transmission of HIV in Sichuan and Yunnan provinces. Methods: Surveys were carried out in drug dependent units or treatment centers in 11 districts or regions with a self-designed questionnaire. Results: A total of 1680 subjects participated in this study with female accounting for 22.3%; 25.1% had senior or above middle school education; 41.9% had no job and 23.4% were farmers; divorce, separation, cohabit accounted for 13.7%, 1.5% and 4.5%, respectively; heroin was the main drug abused (98.4%); 1113 cases (66.3%) ever injected drugs intravenously; 639 cases ever shared needles; the reason for sharing needles was that “drug craving appeared and had no mood to think of others”(50.2%); 1091 cases (64.9%) and 709 cases (42.2%)ever had in their life time or last month sex activities with other than their wives; half of them never used condoms; among 916 responses, 34.8%,28.7% and 11.6% were infected with non-gonococcal urethritis, gonorrhea and condyloma acuminatum, respectively; among 407 cases who had anti-HIV test, 62 cases (15.2%) knew that they were HIV positive. Conclusion: The majority of the subjects had high risk behaviors or experience of HIV infection/transmission such as sharing needles and commercial sexual activities. Some of them had already suffered from STD, hepatitis and HIV. In order to control and prevent the spread of HIV/AIDS, effective comprehensive measures and intervention should be taken immediately.
