合肥地区常见革兰阴性杆菌种类及其耐药性检测

【摘要】 目的 探讨、总结本地区细菌菌种分布及其耐药现状。方法 收集 894株革兰阴性杆菌 ,分析其菌种分布、耐药性。细菌分离、培养按常规方法 ,菌种鉴定采用MicroScanWalkAWay 40、Vitek 32全自动微生物分析仪 ,药敏试验采用微量液体稀释法 ,超广谱 β 内酰胺酶 (ESBLs)检测采用纸片确证试验 ,高产AmpCβ 内酰胺酶 (IB)检测采用Thomson推荐的液体稀释法。结果 在 894株革兰阴性杆菌中 ,分离出大肠埃希菌 340株、铜绿假单胞菌 1 52株、肺炎和产酸克雷伯菌 1 0 1株、不动杆菌 66株。ESBLs检出率 :大肠埃希菌2 8 3 %、克雷伯菌 39 6 % ;IB检出率 :铜绿假单胞菌为45 4%、变形杆菌为 1 3 2 %、枸橼酸杆菌为 72 4%、沙雷菌为 2 7 3 %、肠杆菌属为 49%。在革兰阴性杆菌中 ,亚胺培南、头孢哌酮 /舒巴坦、哌拉西林 /他唑巴坦药物敏感性较高。结论 合肥地区革兰阴性杆菌菌种分布以大肠埃希菌占首位。亚胺培南是治疗革兰阴性杆菌首选药物 ,对于产ES BLs、IB的菌株 ,头孢哌酮 /舒巴坦、哌拉西林 /他唑巴坦可以作为候选药物

【Abstract】 Objective To study distribution of main gram -n egative rods and its resistance to antibiotics in Hefei area. Methods Bacteria were cultured by routine method, identified by MicroScan WalkAWay-40 and Vitek-32 Antibiotic suscep tibility of bacteria was determined by microdilution. Extended spectrum beta lac tamases (ESBLs) were confirmed by disc with CAZ+CD02 and CTX+CD03.High level Amp C β-lactamase was detected by microdilution recommended by Thomson. R esults The ratios of Escherichia coli, Pseudomonas aeruginosa, Klebsiell a, Acinetobacter were higher than others. The rates of ESBLs among Escherichia c oli and Klebsiella were 28 3% and 39 6%.The rates of High level AmpC β-lacta mase among Pseudomonas aeruginosa, Proteus, Citrobacter, Serratia and Enterobact er were 45 4%,13 2%,72 4%,27 3% and 49%. Susceptibility of imipenem was hig her than the others of antibiotics. Conclusion The ratio of Esche richia coli is the highest among all gram-negative rods. Imipenem is the first choice antibiotic in treating infectious diseases which are caused by gram-neg ative rods including ESBLs and high level AmpC β-lactamase. In addition, cefo perazone/sulbactam, piperacillin/tazobactam are secondary choice.
