前列腺病变螺旋CT动态增强扫描的应用研究
Applicative study of spiral CT dynamic-enhanced scan of prostatic diseases
【摘要】 目的 :研究前列腺正常组织、前列腺癌、前列腺增生中央带这三种组织的增强特点 ,根据时间密度曲线求得检出前列腺癌的螺旋CT最佳延迟时间。方法 :采用SIEMENSPLUS 4螺旋CT ,对 10例无泌尿系症状、31例前列腺癌、4 2例前列腺增生患者进行动态增强扫描。对比剂用碘海醇 10 0ml( 30 0mg·I/ml) ,3.0ml/s ,延迟时间 30s,SLICE 5mm。同层动态扫描 2 0层。结果 :正常前列腺组织呈缓慢上升型 ,平均峰值时间为注药后 90s,最佳延迟时间 84~ 96s;前列腺癌均匀增强呈快升慢降型 ,平均峰值时间 6 6s,最佳延迟时间 6 0~ 72s;前列腺增生中央带呈斑片状强化 ,平均峰值时间 70s,最佳延迟时间 6 4~ 76s。结论 :大部分前列腺癌具有早期增强的特点 ,检出癌灶最佳延迟时间为 6 0~ 72s。在此时间内扫描前列腺 ,有利于病灶的检出和定性
【Abstract】 Objective:To study the enhance character of prostatic normal tissue,carcinoma,and prostatic hyperplasia.Using time density curves analysis to define the optimal temporal window for helical CT of the prostate.Methods:10 patients with normal prostate,31 patients with proven prostatic cancer and 42 patients with proven prostatic hyperplasia were studied for time density analysis of the normal tissue,carcinoma and hyperplasia.The total volume of contrast agent was 100ml(300mg·I/ml).a monophasic injection protocol at 3ml/sec was used.20 slices were obtained with dynamic scan method.Results:The contrast enhanced density of normal tissue rise slowly with the time delay,the peak enhancement was 29HU,time to peak enhancement was 90s,cancer appears on helical CT as asymmetric focal or diffuse areas of contrast enhancement in peripheral zone.time to peak enhancement (47HU) was 66s,hyperplasic tissue appears as symmetric contrast enhanced nodules in central gland,time to peak enhancement (38HU) was 70s.Conclusion:Most prostatic cancer show apparent contrast enhancement at early phase of helical CT.The optical temporal window for detect prostatic carcinoma was 60~72s.
