【摘要】 目的 :探讨以水作对比剂的摇篮变换体位CT检查对胃癌手术评估价值。方法 :1 2 8例胃癌病人口服温开水 ,低张法使胃囊充盈应用摇篮技术行CT检查 ,进行术前CT分期并与手术病理结果对照。结果 :1 2 8例胃癌中 1 0 8例术前CT分期与术后临床病理分期相吻合 ,准确率 84.4 %。 1 1例 (8.6 % )CT分期低于临床分期 ,而 9例 (7.0 % )CT高估。CT分期对胃癌手术切除评估准确率 93 .8% (1 2 0 / 1 2 8)。 <1 .0cm和≥ 1 .0cm肿大淋巴结的检出率分别为 2 3 .1 %和 76 .9%。根治术后送检的 1 97个淋巴结中 ,CT发现 1 67个淋巴结 ,敏感性为 84.8% (1 67/ 1 97) ;在 1 1 2个阳性淋巴结中 ,有 98个CT提示转移 ,特异性为 87.5 % (98/ 1 1 2 )。结论 :以水作对比剂摇篮CT扫描明显提高了胃癌术前分期准确率和手术切除评估符合率 ,尤其对显示肿块与周围组织器官结构的关系及肿大淋巴结摇篮体位扫描更具有明显的优越性。更多还原

【Abstract】 Objective:To assess the value of CT with cradle technique and water as oral constrast agent for the preoperative gastric carcinoma.Methods:128 cases of gastric carcinoma underwent CT with new technique before operation. The CT findings were compared with that of the surgical pathology,and staged by CT.Results:108 cases(84.4%)were correctly staged by CT.11 cases(8.6%)were underestimated and 9 cases (7.0%)overestimated on CT staging. The accuracy rate of preoperative evaluation of tumor resectability by CT with new technique was 93.8%(120/128). The rates of nodes discovered by CT were 23.1%and 76.9%,in<1.0cm and≥1.0cm,respectively. Sensitivity was 84.8%(167/197)and specificity was 87.5%(98/112).Conclusion:New CT technique with cradle technique and water as contrast agent greatly improve the preoperative staging and predict the resectability of patints with gastric carcinoma,especially discover apparently the relationship between tumor and adjacent organs or tissues,and intraperitoneal nodes.更多还原
