CT及高分辨率CT在判断肺气肿患者肺间质纤维化程度及预后中的价值

【摘要】 目的 探讨以肺气肿为主的慢性阻塞性肺疾病 (COPD)合并肺间质纤维化的 CT及高分辨率 CT(HRCT)表现。方法 根据确诊的 3 1例肺气肿肺间质纤维化患者的 CT及 HRCT肺气肿程度 ,分析其肺间质纤维化 CT及 HRCT表现。结果 根据 CT及 HRCT表现 ,3 1例中重度肺气肿 19例 ,中度肺气肿 12例 ;肺间质纤维化表现支气管血管束变细 18例 ,支气管壁增厚 5例 ,支气管血管束扭曲、变形 4例 ,小叶内间质增厚 2 1例 ,小叶间隔增厚 2 8例 ,胸膜下线 8例 ,磨玻璃影4例。牵张性支气管扩张 3例 ,蜂窝 7例。结论 胸部 CT及 HRCT对以肺气肿为主的 COPD合并肺间质纤维化的诊断具有重大价值 ,并能为临床诊断和治疗提供重要依据

【Abstract】 ObjectiveTo analysis the CT and high-resolution CT appe arances of chroni c obstructive pulmonary diseases(COPD) with pulmonary interstitial fibrosises wh ich manily present pulmonary emphysema.MethodsThe CT and HRCT were performed in 31 cases pulmonary emphysema patients with pulmonary intersti tial fibrosises diagnosed in our hospital.The CT and HRCT findings of pulmonary interstitial fibrosises were evalued according to the d egree of emphysema.ResultsAccording to their CT and HRCT fin dings, 31 patients were demonstrated as follows:severe emphysema 19 cases,mild e mphysema 12 cases.The pulmonary interstitial fibrosises were demonstrated as f ollows:thinning of bronchovascular bundles 18 case,thickening of bronchial wall 5 case,distortion of bronchovascular bundles 4 cases,intralobular interstitial t hickening 21 cases,interlobular septal thickening 28 cases,subpleural lines 8 ca ses,Groundglass opacity 4 cases,traction bronchiectasis 3 cases,honeycombing 7 c ases.ConclusionCT and HRCT play a important role in diagnosis of COPD with pulmonary interstitial fibrosis which mainly present pulmonary emph ysema.
