高血压患者脉压与心电图、动态心电图异常关系的探讨

【摘要】 目的:研究高血压患者脉压与动态心电图及心电图异常的关系。方法:对339例高血压住院患者行常规12导联心电图及动态心电图检查,以脉压(≤60mmHg,61~80mmHg,81~100mmHg,>100mmHg)4个亚组进行分析。结果:(1)随着脉压(PP)的增加,各种心律失常(除房早、房扑外)的发生率显著增加(P<0.05~<0.001);(2)Logist回归分析:脉压与左室肥厚关系最密切(OR=1.058,P<0.001)。Spearman’s相关分析发现脉压与Rv5相关性最佳(r=0.409,P<0.001)。多元逐步回归分析显示仅脉压与Rv5具有显著回归效果(OR=1.050,P<0.001)。结论:高血压患者脉压与心电图异常相关性密切。

【Abstract】 To investigate the relationship between pulse pressure and the abnormalities of ECG and Holter in patients with hypertension. Methods: We examined ECG and Holter and blood pressure in 339 patients with hypertension. Then we divided them into 4 subgroups according to the level of pulse pressure (≤60mmHg, 61-80mmHg, 81-l00mmHg, >100mmHg) and analyzed these dates. Results: (1) All kinds of arrhythmias (expect atrial premature beats and atrial flutter) increased when pulse pressure increased (P<0. 05-<0. 001) ; (2) Logist analysis showed myocardial hypertrophy correlated best with PP (OR = 1. 050, P<0. 001). Rv5 was correlated best with PP in Spearman’s correlation analysis (r=0. 409, P<0. 001). The multiple factor stepwise regression analysis showed that only the regression of Rv5 with PP was of great significance (OR= 1. 058, P<0. 001). Conclusion: The PP correlated immediately with the abnormalities of ECG and Holter in patients with hypertension.
