慢性充血性心衰患者下丘脑—垂体—甲状腺轴的变化
Changes of hypothalamus-hypophysis-thyroid axial system in patients with chronic congestive heart failure
【摘要】 目的:研究慢性充血性心衰患者甲状腺激素变化与下丘脑-垂体轴系统的关系。方法:应用放射免疫法测定28例慢性充血性心衰(CHF)患者及14例健康人血清甲状腺激素浓度(T3、T4、FT3、FT4、rT3),促甲状腺激素(TSH)浓度和促甲状腺激素释放激素(TRH)浓度。结果:(1)CHF组:T3、FT3、FT4较对照组显著下降(P<0.05~<0.01);(2)随心衰加重,T3、FT3浓度减少,rT3浓度增加,在心衰各组间有显著性差异,而T4、FT4、TSH、TRH水平在心衰各组间无显著性差异。结论:在充血性心衰时,下丘脑-垂体-甲状腺轴系统调节失衡。T3、rT3的改变可作为判断CHF严重程度的指标。
【Abstract】 Objective:To investigate the changes of thyroid hormone and its relationship with hypothalamus-hypophysis axial system in patients with chronic congestive heart failure. Methods: The levels of thyroid hormone (T3、 T4、 FT3、 FT4、 rT3) and thyrotropin (TSH) and thyroliberin (TRH) were measured by radioimmunoassay in 28 cases with CHF and 14 normal controls. Results: (1) The levels of T3, free T3、 free T4 in patients with CHF were significantly lower than those of normal controls (P<0. 05. P<0. 01, and P<0. 05), the level of rT3 in CHF patients was significantly higher than that of control group (P<0. 01). But there were no differences in TSH and TRH levels between patients and controls (P>0. 05); (2) With the heart failure deteriorated, the levels of T3 and free T3 lower and the levels of rT3 higher, but the levels of T4, free T4, TSH, TRH were not related to the cardiac function. Conclusion: The balance in hypothalamus-hypophysis-thyroid gland axial system was broken in patients with CHF. The heart function of CHF may be judged by the changes of T3 and rT3.
