【摘要】 目的探讨改良式剖宫产术的临床价值。方法于下腹部两髂前上棘连线下 2～ 3cm处做横行直切口 ,钝性分离脂肪层及腹直肌 ,横向撕开腹膜 ,肠线单层连续缝合子宫肌层 ,不缝合腹膜 ,肠线连续缝合筋膜及间断全层缝合皮肤及皮下组织。并与同期腹部横弧形切口剖宫产术和子宫下段剖宫产术进行比较。结果改良式手术时间短 ,术中娩头困难发生率低 ,出血量少 ,术后排气时间缩短 ,疼痛轻 ,病率低 ,住院时间短。与其它两组比较差异有显著性 (P <0 .0 5 ) 。结论改良式剖宫产术手术时间短 ,术后并发症少 ,住院时间短 ,值得临床推广应用更多还原

【Abstract】 Objective To explore the clinical value of modified cesarean section.Methods A transverse straight incision was performed at lower abdomen 2～3cm below the level of anterior superior iliace spine,the fat layer and muscular rectus abdominis were seperated bluntly,the peritoneum was torn transversely.Myometriam was continuously sutured by catgut,while peritoneum wasn′t sutured.Fascia layer was continuosusly sutured by catgut,and the skin and subcutaneous tissue were sutured interruptedly.We had made a comparison between the modified cesarean section and the operation with transverse arc incision at lower abdomen or the lower uterine segment cesarean section at the same period.Results The modified cesarean section had many advantage such as shorter operative time,lower rate of difficult delivery of fetal head and less blood loss during operation,shortening of exsufflation time,slight pain and lower sickness rate after operation and less days in hospital.The difference was significant as compared with the lther two groups(P<0.05).Conclusion Modified cesarean section should be spread in clinic for its advantages of shorter operative time,less postoperative complications and less days in hospital.更多还原
