【摘要】 目的比较米索前列醇口服与阴道两种给药方式用于足月妊娠引产的效果和安全性。方法将5 1 0例有引产指征的足月产妇随机分为口服 ( 2 62例 )与阴道用药 ( 2 48例 )两组。口服组 1 0 0 μg ,阴道组5 0 μg(阴道后穹窿放药 ) ,每 4小时给药 1次 ,直到出现规律宫缩。 结果阴道组在引产有效率、临产发动时间、用药次数等方面均明显优于口服组 ,而宫缩过频过强的发生率 ,急产、剖宫产率均明显高于口服组 ,阴道分娩率明显低于口服组。结论米索前列醇用于足月妊娠引产口服、阴道用药均有效 ,但口服给药阴道分娩率高 ,对母儿影响小 ,用药方便、安全 ,明显优于阴道给药更多还原

【Abstract】 Objective To compare the safety and efficacy of intravaginal and oral administration of misoprostol for labor induction at term.Methods 510 patients with indication for induction of term labor (gestational age :37～41 + weeks) were randomly allocated into 2 groups.In group A (n=262) misoprostol (100μg) was orally administered and group B (n=248) the drug (50μg) was placed in the posterior vaginal fornix.Misoprostol was given every 4 hours for a maximum of four doses until adequate labor was achieved (at least three contractions in 10 minutes).The effectiveness was considered that labor had been induced within 24 hours after last administration the misoprostol.Fetal heart rate and contraction was uninterruptedly monitored at onset of inducing labor.Data was stored in computer.Results In comparison with group A (per os),in the group B (per vaginam) the induction efficacy was better (P<0.05),the length for labor onset was shorter (P<0.01),and the times of administration was less,but the incidance of overstrength and overfrequency of contraction (P<0.01),the rate of partus pricipitatus and caesareus were obviously higher (P<0.05),whereas the rate of partus per vaginam was remarkably lower (P<0.05).Conclusion Both peroral and pervaginal administration of misoprostol are effective for labor induction,but the peroral methd will bring a higher rate of partus per vaginam,a convenient procedure and a better security for mother and child,so it is more superior than method per vaginam.更多还原
