【摘要】 目的 :探讨四肢开放性骨折的治疗方法与疗效。方法 :对我院 1999～ 2 0 0 0年 2年间共计 88例四肢开放性骨折病例治疗过程及结果进行回顾分析。结果 :GustiloⅠ型一期愈合 34例 ,GustiloⅡ型一期愈合 12例 ,另 7例延期愈合疗效满意 ,GustiloⅢa19例一期清创植皮加外固定架治疗 ;Ⅲb 型 9例利用周围软组织覆盖加外固定架治疗一期愈合 ,2例二期清创植皮成功 ;ⅢC 型 5例截肢 3例 ,1例血管栓塞后截肢 ,1例反复骨髓炎。结论 :对于开放性骨折治疗应严格执行Rob ert制定的关于治疗开放性骨折治疗原则。更多还原

【Abstract】 Objective: To discuss the surgical treatment for open fractur e of extre mities and its efficacy. Methods: Eighty patients with open limbs fractures hos pitalized during 1999-2000 , were retrospectively analyzed in terms of fracture t ypes and therapeutic course and its efficacy. All patients were treated based on the general principles for treating open limbs fracture. Results: Thirty four p atients with Gustilo grade I fractures and 12 cases with Gustilo grade Ⅱ fractu res got primary healing, while 7 others with Gustilo grade Ⅱ fractures got dela yed healing with satisfied therapeutic efficacy. Nineteen cases with Gustilo gra de Ⅲa fractures got primary healing following debridement, skin grafting and e xternal fixation,9 cases with grade Ⅲb fractures got primary healing after cove ring the wound with surrounding soft tissue and external fixation while 2 others with grade Ⅲb fracture got second intention following debridement and skin gra fting. Out of 5 cases with Gustilo gradeⅢc fractures, 3 cases were cured by amp utation, 1 case was amputated due to thrombosis again and the remaining one case was also amputated due to repeated osteomyelitis. Conclusion: The principles fo r treating open fracture, put forward by Robert, should be strictly followed for obtaining satisfied therapeutic efficacy.更多还原
