前部缺血性视神经病变患者眼血流动力学检测

【摘要】 目的 探讨前部缺血性视神经病变 ( anterior is-chemic optic neuropathy,AION)患者眼血流动力学的变化 ,及血管因素对 AION视功能损害的影响。方法 应用彩色多谱勒血流成像技术 ( color Doppler flow im aging,CDFI) ,观测前部缺血性视神经病变 32例 38眼眼睫状后动脉 ( poste-rior ciliary artery,PCAs)、眼动脉 ( ophthalmic artery,OA)血流动力学的变化 ,并与正常对照组进行比较。结果 AION患者 OA的收缩期峰值速度 ( peak systolic velocity,PSV) ,舒张末期速度 ( end- diastolic velocity,EDV)平均值分别为 ( 2 8.87± 3.11) cm· s- 1、 ( 12 .73± 1.38) cm· s- 1 ,阻力指数 ( resistance index,RI)为 0 .72± 0 .0 2 ;PCAs的PSV、EDV分别为 ( 6 .10± 1.5 5 ) cm· s- 1 ,( 1.96± 0 .19) cm· s- 1 ,阻力指数为 0 .6 7± 0 .0 1。同对照组比较 ,PCAs、OA的 PSV,EDV明显降低 ,RI明显增高 ,其差异有非常显著性意义 ( P<0 .0 0 1)。结论  AION患者的 OA、PCAs血流量下降 ,血管阻力增加。CDFI对 AION的诊断、治疗效果的评估有较大的应用价值

【Abstract】 Objective To investigate the retrobulbar hemodynamics of patients with anterior ischemic optic neuropathy(AION) and the effect of vascular factor on the deterioration of visual function in AION .Methods Thirty eight eyes ( 32 patients )with AION clinically diagnosed were studied by using color Doppler flow imaging( CDFI). The peak systolic velocity (PSV), end diastolic velocity (EDV), and resistance index (RI) of the posterior ciliary arteries (PCAs) and ophthalmic artery (OA) were detected.Thirty eyes of 15 normal were the control group.Results In the comparison with the control group, PSV, EDV of OA in patients with AION declined (28.87±3.11) cm·s -1 , (12.73±1.38 ) cm·s -1 , respectively and RI of OA increased 0.72±0.02.PSV,EDV of PCAs declined (6.10±1 55) cm·s -1 , (1.96±0.19 ) cm·s -1 , respectively and RI of PCAs increased 0.67±0 01,there are significant differences ( P <0.001).Conclusion The blood flow of retrobulbar arteries of the patients with AION decreases significantly, CDFI is a valuable method to diagnosis AION and to evaluate the curative effect of the treatment of AION.
