【摘要】 [目的]探讨术前应用顺铂放化疗治疗Ⅱa~Ⅱb1期巨块型宫颈癌的临床疗效。[方法]对161例Ⅱa~Ⅱb1期宫颈癌的治疗进行回顾性分析,其中放化疗+手术(放化疗组)48例,放疗+手术组(放疗组)55例,单纯手术组(手术组)58例。对3组的近期疗效、组织学改变及预后情况进行比较。[结果]放化疗组近期疗效(93.75%)明显高于放疗组(80%),具有显著性差异(P<0.05)。放化疗组深肌层癌细胞损伤率为70%,明显高于放疗组(43.6%)及手术组(24.1%),具有显著性差异(P<0.01)。局部复发放化疗组为4.16%,放疗组为13.72%,手术组为20.68%,放化疗组与其他两组比较均具有显著性差异(P<0.05)。5年生存率放化疗组83.4%,放疗组74.86%,手术组55.82%,放化疗组与手术组比较具有显著性差异(P<0.05)。[结论]术前放化疗可提高巨块型宫颈癌疗效,增加手术机会,提高生存质量。更多还原

【Abstract】 To evaluated the clinical effic acy of preoperative chemoradiotherapy with cisplatin on stageⅡa andⅡb1bulky ce rvical carcinoma.One hundred and sixty-one cases with stageⅡa andⅡb1 cervical car-cinoma were analyzed.The patients were divided into3group:group1,c hemoradiation and surgery(preoperative chemoradiation with cisplatin and brachyt herapy,radical hysterectomy and pelvic lymphadenectomy,n=48);group2,radiotherapy and surgery(preoperative radiation with brachytherapy,radical hysterectomy and pelvic lymphadenectomy,n=55)and group3,sugery alone(radical hysterectomy and pel vic lymphadenectomy,n=58).Short-term effect,histologi-cal alteration and progn osis among the three arms were compared.The efficacy of group1(93.75%)w as higher than that of group2(80%)with significant difference(P<0.05).The rate o f cancer cells lethal damage in deep stroma was70%in group1.It was obviously hig her than that in group2(43.6%)and group3(24.1%)with significant differ-ences(P< 0.01).The recurrent rates of them were4.16%,13.72%and20.68%respectively.5-year survival rates of group1,2,3were83.4%,74.86%and55.82%respectively.Significant difference(P<0.05%)was found between group1and group3.[Conclusion]Preoperative c hemoradiation can enhance the efficacy of bulky cervical carcinoma.The pos-sibi lity of operation increases.The quality of life is also improved.更多还原
