小儿锌、铁、钙营养状况与幽门螺杆菌感染的关系
HELICOBATER PYLORI INFECTION AND DEFICIENCIES OF ZINC,IRON AND CALCIUM IN PRESCHOOL CHILDREN.
【摘要】 目的 :探讨幽门螺杆菌感染 (H. pylori)对小儿微量元素锌、铁和钙营养状况的影响。方法 :选择 479例无症状学龄前儿童进行头发微量元素锌、铁、钙含量测定和血清 H. phlori抗体检测。对小儿铁、锌、钙缺乏与可能影响小儿微量元素营养状况的相关因素行进行单因素 χ2分析 ,筛选出相关因素。以此为基础 ,进行多因素 logistic回归分析。结果 :单因素分析表明 :H.pylori感染组小儿发铁缺乏率高于未感染组 ,而发锌、发钙缺乏率在两组之间差异无显著性。以非条件 lo-gistic多元回归模型有效控制其他因素的混杂偏倚后 ,H.pylori感染仍是发铁缺乏的主要危险因素 ,其 OR值为 1.93 (OR95 %CI:1.0 3~ 3 .62 )。结论 :幽门螺杆菌感染是造成或加重机体铁营养不良的因素 ,对 H.pylori感染患儿有必要进行头发微量元素含量检测 ,及时给予适当的治疗 ,以减少由此引的铁缺乏对小儿生长发育的危害
【Abstract】 Objective:To evaluate the effect of Helicobacter pylori(H.pylori) infection on preschool children status of zinc,iron and calcium Methods:Measure ser um antibody to H pylori and hair contents of zinc,iron and calcium among 479 preschool childre n w ithout symptoms of any diseases The effects of risk factors were anal y sed using univariate factor analysis and unconditional logistic regression analy sis Results:H pylori seropositi ve group had a higher prevalence of hair iron deficiency than the H pylori seronegative group;There w as no markable difference in the prevalence of hair zinc deficiency between two gr oups,so was the prevalence of calcium deficiency After effectively cont rolling t he confounding bias from other factors with unconditional logistic regression,H pylori infection remained to be the mainly risk factor of hair iron deficiency,t he odds ratio(OR) was 1 93(95%CI:1 03~3 62) Conclusion:H p ylori infection is the factor that leads to or aggravates iron deficiency. In order to dimin ish the damage of iron deficiency to childrens growth and development,it is n ec essary for the children with H pylori infection to have an examination of hair trace-element status and accept a proper treatment in time
