兔心肌缺血再灌注心电图分析

【摘要】 目的探讨兔心肌缺血40min再灌注40min心电图变化。方法采用兔缺血再灌注模型10只,观察心电图Ⅰ、Ⅱ导联ST段、T波、QRS波群的变化及心律失常的发生情况。结果①ST-T改变:9只在结扎即刻~20minST段、T波进行性抬高,此后相对恒定;再灌注早期ST段、T波快速回落,8只20min内ST段回落>50%。②QRS波群改变:5例在缺血早期5min内出现R波增高(急性损伤阻滞),4只在缺血期出现R波降低或q波加深(其中1只R波降低同时出现终末r’波)。③心律失常发生情况:缺血期1只发生室性期前收缩;再灌注期3只发生室性心律失常。结论兔急性心肌缺血早期常表现为ST段抬高、T波增高;再灌注早期ST段快速回落,20min内ST段回落>50%,可作为兔冠状动脉再通的指标。

【Abstract】 Objective To explore changes of electrocardiogram of myocardial ischemia 40 min and reperfusion 40 min in rabbits. Methods To utilize 10 models of myocardial ischemia and reperfusion and observing changes of ST segment, T wave, ORS complex and rises of arrhythmia about electrocardiogram of lead Ⅰand Ⅱ. Results ST segment and T wave of 9 cases elevated gradually from coronary artery occlusion to ischemia 20 min. They were stable relatively during ischemia 20~40 min and depressed rapidly in forepart of reperfusion, depression of ST segment of 8 cases is more than 50% in reperfusion 20 minutes. R wave of 5 cases elevated in forepart of ischemia during 5 min, ie. acute injury block. Depression of R wave or deepening q wave of 4 cases occurred during ischemi one case appeared r’ wave at end of QRS. One case appeared ventricular premature beat during ischemia. Three cases appeared ventricular arrhythmia during reperfusion. Conclusion Results suggest that elevation of ST segment and T wave is general phenomenon during forepart of acute myocardial ischemia.ST segment is depressed rapidly during forepart of reperfusion, depression of ST segment > 50% in reperfusion 20 min can be regarded as index of coronary artery reflow of rabbit.
