【摘要】 目的 :探讨宫腔镜双极汽化切割手术治疗子宫内病变的疗效。方法 :为前瞻性临床研究。 2 0 0 1年 3月至 2 0 0 2年 7月应用宫腔镜双极汽化进行宫腔内电手术 86例 ,其中子宫内膜息肉 4 2例 ,粘膜下子宫肌瘤 2 0例 ,功能性子宫出血 19例 ,子宫粘连 3例和不全子宫纵隔 2例。结果 :所有手术均成功完成 ,摘除子宫息肉 (TCRP) 35例 ,TCRP +去子宫内膜术 (TCRE) 7例 ,切除粘膜下子宫肌瘤 (TCRM) 18例 ,TCRM +TCRE 2例 ,TCRE19例 ,分解子宫内粘连 3例和切开子宫中隔 2例。平均手术时间范围为 (2 2 .2± 7.9)～ (6 5 .0± 7.1)min。6 9例术前诉有月经过多的病人中 ,经TCRE、TCRP、TCRP +TCRE、TCRM和TCRM +TCRE术后其症状缓解率分别为 10 0 %、93.3%、10 0 %、88.9%和 10 0 %。结论 :采用以生理盐水膨宫的宫腔镜双极汽化切割手术易于操作、安全 ,能达到去除子宫内病变的疗效更多还原

【Abstract】 Objective:To study the effects of hysteroscopic bipolar electrosurgeries on treatment of intrauterine lesions.Methods:Prospective clinical study was carried out in 86 women mainly complaining about heavy bleeding, including endometrial polyps (42 cases), submucous myomas (20 cases), dysfunctive uterine bleeding (19 cases), intrauterine adhesions (3 cases) and subseptate uterus (2 cases), which were recruited between March, 2001 and July,2002.Results:All the scheduled operation could be done successfully with the new devices,including 35 cases of removing polyps (TCRP), 7 cases of TCRP+endometrial ablation (TCRE), 18 cases of resecting submucous myomas (TCRM), 2 cases of TCRM+TCRE, 19 cases of TCRE, 3 cases of lysing intrauterine adhesions and 2 cases of incising uterine septa, the range of mean operating time was 22.2±79～65.0±7.1 min.In 69 patients with heavy bleeding, the percents of symptom relief were 100%, 93.3%, 100%, 88.9% and 100% after TCRE, TCRP, TCRP+TCRE, TCRM and TCRM+TCRE respectively.Conclusion:The hysteroscopic bipolar electrosurgery is easy to handle, and safe and effective in removing intrauterine lesions.更多还原
