炎症性肠病患者肠黏膜菌群改变及抗体反应

【摘要】 目的 探讨炎症性肠病 (1BD)患者肠道菌群改变及机体免疫反应。方法 采用梯度稀释法行菌群分析 ,同时采用透射比浊法测定IgG、IgM、IgA。结果 发现急性期溃疡性结肠炎患者菌群中肠杆菌 (8 82± 0 69,P <0 0 5 )、肠球菌 (7 73± 0 2 1,P <0 0 1)及小梭菌 (6 68± 0 78,P <0 0 1)的数量显著增加 ,而双歧杆菌 (6 89± 0 3 4,P <0 0 1)和乳杆菌 (6 95± 0 5 2 ,P <0 0 1)的数量明显下降 ,而缓解期与对照组相比无明显差异。拟杆菌 (7 2 6± 0 0 3 ,P <0 0 5 )及双歧杆菌 (7 5 9± 0 3 4,P <0 0 1)较急性期明显上升 ,小梭菌 (6 13± 0 66,P <0 0 1)明显下降。急性期克隆氏病患者的菌群中肠杆菌 (8 93± 0 62 ,P <0 0 1)和酵母菌 (3 88± 0 96,P <0 0 5 )明显增加 ,而双歧杆菌 (6 44± 0 2 5 ,P <0 0 1)和乳杆菌 (6 87± 0 43 ,P <0 0 1)明显下降。缓解期较正常菌群相比肠杆菌 (8 82± 0 42 ,P <0 0 5 )仍很高 ,而双歧杆菌 (7 11± 0 3 4,P <0 0 1)有回升但仍明显低于正常。缓解期与急性期相比双歧杆菌的数量有明显差异。监测的血清抗体反应中 ,急性期UC病人比正常组有更高的IgG(17 92± 3 62 ,P <0 0 1) ,IgA也有上升。而在缓解期IgA(2 84± 0 76,P <0 0 5 )与正常组相?

【Abstract】 Objective To investigate the changes of fecal flora and humoral immunofunction with Inflammatory bowel disease patients Methods The fecal flora is analyzed by gradient dilution method and we measured the humoral immunofunction of them by transmission turbidmetry at the same time Results The Enterobacteria (8 82±0 69, P < 0 05 )?Enterrococcus (7 73±0 21, P <0 01) and Clostridium (6 68±0 78, P <0 01) increased significantly in acute UC patients compared with controls,while Bifidobacterium (6 89±0 34, P <0 01) and Lactobacillus (6 95±0 52, P <0 01) reduced sharply At the inactive phase of UC patients,no great changes contrasted with the controls Bacteroides (7 26±0 03, P <0 05) and Bifidobacterium (7 59±0 34, P <0 01) ascended significantly at the inactive phase of UC than that of active phase Clostridium (6 13±0 66, P <0 01)decreased simultaneously Comparison between CD patients and controls revealed greater numbers of Enterobacteria (8 93±0 62, P <0 01) and Yeast (3 88±0 96, P <0 05) at the active phase CD and of Bifiobacterium (6 44±0 25, P <0 01) and Lactobacillus (6 87±0 43, P <0 01) in controls Furthermore,the concentrations of mucosal Enterobacteria increased progressively with the severity of disease At the same time,there is a disorder in humoral immunofunction At the acute phase of IBD,IgG increased greater than that of controls,and IgA escalated at inactive phase Conclusions The results can provide a scientific basis for clinical therapy on the inflammatory bowel diseases,and might assess the prognosis of it
