激光周边虹膜切除术预防闭角型青光眼急性发作临床观察

【摘要】 目的 :观察急性闭角型青光眼的对侧眼行预防性激光周边虹膜切除术的有效性和安全性。方法 :30例 (30眼 )急性闭角型青光眼的对侧眼行氩激光联合Nd:YAG激光预防性激光周边虹膜切除术 ,对治疗前后的眼压、前房深度、前房角、视野及UBM情况进行观察对比。随访时间为10~12月。结果 :在随访期内治疗前后视力无变化 ;28眼 (占93.3% )未用降眼压药物 ,眼压控制良好。已有房角粘连、眼底及视野损害的2眼 (占6.7% ) ,眼压仍高于21mmHg ,需用抗青光眼药物治疗 ;大部分病例房角较前增宽 ,周边虹膜拥挤现象减轻 ,房角粘连范围减小 ;无严重术后并发症。结论 :激光周边虹膜切除术对于无广泛房角粘连和关闭的早期闭角型青光眼有预防发作和治疗作用 ,使用氩激光联合Nd:YAG激光安全可靠

【Abstract】 Objective:To observe the efficiency and safety of on the fellow eyes of primary acute angle Closure glaucoma who underwent prophylactic laser peripheral iridotomy.Methods:Prophylactic laser peripheral iridotomy was performed with combined argon and Nd:YAG laser on the fellow eyes of30cases(30eyes)suffering from acute attack of angle Closure glaucoma,pre-and post operatively,IOP,anterior chamber depth,anterior chamber angle,visual field and UBM were studied.The follow-up period was10~12months.Results:All30cases had stable visual acuity,the IOP in28cases(93.3%)were successfully controlled without anti-glaucoma medication.Two cases(6.7%)with pre-existing chronic angle Closure glaucoma had IOP higher than21mmHg required additional glaucoma medications.In most of the cases,anˉterior chamber depth increased,chamber angle widened and synechiae diminished.No severe complication was found.Conclusion:Prophylactic laser peripheral iridotomy with combined arˉgon and Nd:YAG laser is a safe and effective method for treatment of early stage of glaucoma without extensive anterior chamber angle synechiae and closure.
