【摘要】 目的比较应用不同方法矫正尺神经损伤后环、小指爪形指畸形及重建示指外展功能。方法对34例尺神经损伤后的手内在肌麻痹行不同术式的功能重建,其中环、小指爪形指畸形矫正26例中行掌指关节囊紧缩、滑车前移术22例,环指浅屈肌腱移位重建蚓状肌功能4例;示指外展功能重建13例中行示指固有伸肌腱代第一背侧骨间肌10例,掌长肌-掌腱膜代第一背侧骨间肌3例。结果环、小指爪形指畸形的矫正:行关节囊紧缩、滑车前移术,优良率95%;指浅屈肌腱代内在肌,优良率75%。两种示指外展功能重建术优良率均为100%。结论环、小指爪形指畸形的矫正行关节囊紧缩、滑车前移术,方法简单,疗效肯定;而示指外展功能重建的两种方法均可。更多还原

【Abstract】 Ob je ctive To compare various corrective procedures of the ulnar nerve palsy clawing deformity of the ring and little fingers and abductor paralysis of the index fin ger.Methods Thirty-four cases of intrinsic muscles paralysis in the hand cause d by ulnar nerve injury were corrected with various surgical procedures.26cas es of clawing of ring and little fingers were treated by metacarpophalangeal joi nt capsulodesis in22cases,ring superficial flexor tendon transfer in4cases. 13cases of abductor paralysis of index finger were treated by extensor indicis proprius transfer to substitute for the first dorsal interosseous muscle in10 cases,palmaris longus-aponeurosis transfer to substitute for the first dorsal interosseous muscle in3cases.Results The results were excellent or good in95 %of the patients treated by capsulodesis to correct clawing deformity of the ri ng and little fingers,and75%of the superficial flexor tenton transfer.But i t was100%,which in both of reconstruction of abductor paralysis of index fi nger.Conclusion The method of metacarpophalangeal joint capsulodesis is simple and valid for correcting the clawing deformity of the ring and little fingers.B oth extensor indicis proprius transfer and palmaris longus-aponeurosis transfer are valid to recon-struct the index finger abduction.更多还原
