拜唐苹控制2型糖尿病餐后高血糖的临床观察

【摘要】 目的 观察拜唐苹控制 2型糖尿病餐后高血糖的疗效。方法 对 36例 2型糖尿病患者 ,在合理饮食、运动和口服磺酰脲类 二甲双胍降血糖药治疗基础上 ,空腹静脉血糖≤ 7.0mmo1 L ,餐后 2h静脉血糖≥ 11.1mmol L者加服拜唐苹 50mg ,一日 3次 ,观察 3周。观察期前、中、后各测空腹静脉血糖和胰岛素早餐后 2h静脉血糖和胰岛素并监测血脂和肝、肾功能。结果 服用拜唐苹后 ,对降低餐后高血糖的疗效评定 :显效 9例 ( 2 5.0 % ) ;有效 2 4例 ( 6 6 .7% ) ;弱效 3例 ( 8.3% ) ,总有效率达 91.7%。空腹血糖稍有降低和血胰岛素浓度无升高 (P >0 .0 5) ,血脂异常有改善 (P <0 .0 1)。观察中部分患者有一过性轻度腹胀、腹痛 ,无低血糖 ,无肝、肾功能损害。结论 拜唐苹控制餐后高血糖效果明显 ,不增加胰岛 β细胞负荷 ,可改善血脂异常 ,有助于糖尿病二级、三级综合性防治

【Abstract】 Objective To observe the effect of Acarbose on controlling the postprandial hyperglycemia of the Type-2 diabetes mellitus. Methods Those patients(fasting venous plasma glucose ≤7 mmol/L and 2-hour postprandial venous plasma glucose≥11.1mmol/L)in all the 36 Type-2 diabetes mellitus patients were also given oral Acarbose(50mg,tid) for 3 weeks besides rational dietary, sports, and oral sulfonylureas/metformin. The fasting and 2-hour post-breakfast venous plasma glucose and insulin was measured before, during and after the 3-week observation period. The plasma lipoid,liver and kidney function were also measured .Results The total effectiveness rate on controlling postprandial hyperglycemia after taking oral Acarbose amounted to 91.7%. significantly effective for 9 patients(25.0%),effective for 24(66.7%),and weakly effective for 3(8.3%).The level of fasting plasma glucose and insulin before and after taking oral Acarbose were insignificantly different( P >0.05),and with improvement in dyslipidemia situation( P <0.01).Apart from transient abdominal distention and bellyache in some patients, hypoglycemia , liver and kidney impairment was not found.Conclusion Acarbose has significanteffect on controlling postprandial hyperglycemia, does not add burden to pancreatic islets β cell, improve the dyslipidemia situation and will contribute to the secondary and tertiary prevention of diabetes mellitus.
