肾上腺髓质素在川崎病冠状动脉炎中的作用
An experimental study on the role of adrenomedullin in kawasaki disease
【摘要】 目的 探讨肾上腺髓质素 (AM)在实验性家兔川崎病 (KD)并冠状动脉炎中发挥的作用 ,同时检测血浆内皮素 1(ET 1) ,并进行比较研究 ,分析内皮源性血管活性物质AM和ET 1的关系及临床意义。方法 用牛血清清蛋白复制幼兔川崎病动物模型 ,35只幼兔分为实验组 2 5只和正常对照组 10只 ,分别检测血浆AM及ET 1浓度 ,作冠状动脉造影检查 ,并取冠状动脉组织作病理学分析。结果 2 5只实验组幼兔中 6只冠状动脉扩张。 1.血浆AM在无冠状动脉扩张组较正常对照组升高 ,差异有显著性 (P <0 .0 5 ) ;冠状动脉扩张组较无扩张组明显升高 ,差异有极显著性 (P <0 .0 1) ;实验组与正常对照组比较差异有极显著性 (P <0 .0 1)。 2 .血浆ET 1在无冠状动脉扩张组较正常对照组升高 ,差异有显著性 (P <0 .0 5 ) ;冠状动脉扩张组比无扩张组升高 ,差异有显著性 (P <0 .0 5 ) ;实验组与正常对照组比较差异有极显著性 (P <0 .0 1)。结论 1.内皮源性血管活性物质AM和ET 1共同参与KD的病理发病机制 ,在KD急性期相互作用、相互协调 ,在KD的病理生理过程中发挥重要作用 ;2 .血浆AM在KD急性期是作为一种保护因子代偿性升高 ,有冠状动脉扩张时升高更明显 ,推断AM检测可作为KD预测并发冠状动脉炎尤其是冠状动脉扩张的生化指标之一
【Abstract】 Objective To study the role of adrenomedullin(AM) in kawasaki disease(KD) of rabbits and the inflammation of coronary artery, to discuss the role of enthelin 1(ET 1) ,and to study the interaction of AM and ET 1.Methods An experimental model of weanling rabbits for KD was reproduced by bovine serum. There were 35 weanling rabbits , 10 as controls and 25 as experimental ones. We measured the concentrations of AM and ET 1 respectively. The coronary arteriography and pathological analysis of coronary artery were also done.Results Dilatation of coronary artery appeared in 6 rabbits of the experimental group.1.The plasma AM levels in coronary artery group without dilatation were higher than those in control group(P<0.05); the plasma AM levels in the group with dilatation of coronary artery were apparently higher than the group without dilatation of coronary artery group(P<0.01); the plasma AM levels in experimental group were apparently higher than those in control group(P<0.01). 2.The plasma ET 1 levels in group without dilatation of coronary artery were higher than those in the control group(P<0.05); the plasma ET 1 levels in the group with dilatation of coronary artery were higher than those in the group without dilatation of coronary artery (P<0.05); the plasma ET 1 levels in experimental group were apparently higher than those in the control group(P<0.01).Conclusions 1.AM and ET 1 synergically take part in the pathogenesis of KD. They have interaction and coordination, and have an important role in the pathophysiological process of KD.2.The plasma AM levels compensatively increase as a protective factor in the acute phase of KD, and it becomes more apparently when there is dilation of coronary artery. So it is suggested that AM may act as a marker to predict the inflammation of coronary artery especially the dilatation of coronary artery.
