腰椎骨质疏松双能X线吸收法骨密度测定与MRI对照分析
Lumbar Vertebral Osteoporosis:Comparative Analysis BetweenMRI and Dual Energy X-ray Absorptiometry
【摘要】 目的 探讨腰椎骨质疏松 (LVOP)MRI表现与骨矿物密度 (BMD)的关系。方法 65例LVOP病例先作腰椎双能X线吸收法BMD测定予以确诊 ,后进行腰椎SE序列MRI扫描 ,就其MRI征象与BMD作对照分析。结果 MRI未发现信号异常的LVOP其平均BMD高于信号异常者的平均BMD(Ρ <0 .0 1) ;合并椎体疏松性骨折的LVOP其平均BMD低于未合并椎体疏松性骨折者的平均BMD(Ρ <0 .0 1) ;伴发椎体终板疝的LVOP其平均BMD低度于未伴发椎体终板疝者的平均BMD(Ρ <0 .0 5 )。合并有不同类型椎体疏松性骨折的LVOP之间平均BMD的差异无显著性 ,单发椎体疏松性骨折与多发椎体疏松性骨折的LVOP之间平均BMD的差异亦无显著性。结论 轻度的LVOP其MRI信号变化不明显 ,合并椎体疏松性骨折者多为较严重的LVOP ,LVOP伴发的椎体终板疝与腰椎BMD有关
【Abstract】 Objective To study the relations between manifestations of magnetic resonance imaging(MRI)of lumbar vertebral osteoporosis (LVOP) and its vertebral bone mineral density(BMD).Methods After diagnosis of LVOP were confirmed by dual energy X-ray absorptiometry,lumbar vertebrae of 65 patients were examined by SE sequence MRI.Comparative analysis was done between signs of MRI of LVOP and vertebral BMD.Results The vertebral mean BMD of LVOP patients whose changes of signals could not be detected on MRI was higher than that of LVOP patients whose changes of signals were detected on MRI(Ρ<0.01).The vertebral mean BMD of LVOP with vertebral body osteoporotic fractures was lower than that of LVOP without vertebral body osteoporotic fractures(Ρ<0.01).The vertebral mean BMD of LVOP with vertebral body end-plate hernia was also lower than that of LVOP without vertebral body end-plate hernia(Ρ<0.05).There was not significant difference of the vertebral mean BMD among LVOP with different types of vertebral body osteoporotic fracture,and there was also not significant difference of the vertebral mean BMD between LVOP with single vertebral body osteoporotic fracture and LVOP with multiple vertebral body osteoporotic fracture.Conclusion The signal changes of mild LVOP on MRI were not obvious.Most of LVOP with vertebral body osteoporotic fracture were moderate or severe LVOP.Vertebral body end-plate hernia of LVOP related to lumbar vertebral BMD.
