【摘要】 目的　分析青海省西宁胃癌高发区 ,幽门螺杆菌 (Hp)特异性蛋白CagA、VacA、UreA及UreB的阳性率 ,及其与该地区患者胃镜大体诊断和病理诊断间的关系。方法　行胃镜检查时 ,内镜下取组织送病理检查 ,并进行CLO、WS及WesternBlot检测 ,判断其Hp的感染情况。通过WesternBlot的方法判断患者感染的Hp中CagA、VacA、UreA及UreB的情况。 结果　在内镜大体诊断的比较中 ,UreA的阳性率在慢性浅表性胃炎组中明显低于胃、球部溃疡和复合溃疡组 ,亦低于慢性萎缩性胃炎、胃溃疡及胃癌组。而UreB的阳性率在胃、球部溃疡及胃溃疡中显著低于慢性浅表性胃炎、萎缩性胃炎 ,有显著性差异。UreB在肠上皮化生、萎缩性胃炎、胃癌组中的阳性率显著低于慢性浅表性胃炎及消化性溃疡组 ,且在慢性浅表性胃炎中显著升高。CagA、VacA在不同内镜诊断或病理诊断的标本中的阳性率无差异。结论　西宁胃癌高发区Hp菌株的UreA可能与患者发生较严重的胃病变(慢性萎缩性胃炎、消化性溃疡及胃癌 )有关 ,而UreB可能对胃的严重病变 (消化性溃疡及胃癌 )有益更多还原

【Abstract】 Objective To analyze the positive rate of the special protein CagA,VacA,UreA and UreB of Helicobacter pylori in the district with a high incidence of gastric carcinoma in Xinin,Qinghai.The relation of the positive rate and diagnosis is also studied.Methods The gastric tissue which was taken through endoscopy was examined by CLO,WS and Western Blot to detect Hp′s infection.The positive rate of Hp′s CagA,VacA,UreA and UreB in patients with Hp infection was examined.Results The positive rate of UreA was significantly lower in chronic superficial gastritis than in duodenal ulcer and compound ulcer,as well as chronic atrophic gastritis,gastric ulcer and gastric cancinoma.However,the positive rate of UreB was notably lower in duodenal ulcer and gastric ulcer than in chronic superficial gastritis,atrophic gastritis.UreB was notably lower in intestinal change,atrophic gastritis and gastric cancinoma than in chronic superficial gastritis and ulcers.UreB increased markedly in chronic superficial gastritis.CagA,VacA were not significantly different between all the diagnosis.Conclusion Hp′s UreA may correlated with serious gastric disease.However,UreB may have protective effect for serious gastric disease.更多还原
