兔分级脊髓损伤后运动诱发电位的变化
Changes of Motor Evoked Potential in Graded Spinal Cord Injury in Rabbits
【摘要】 目的 了解不同程度脊髓损伤(spinal cord injury,SCI)与运动诱发电位(motor evoked potential,MEP)变化之间的关系以及MEP在判断运动预后功能中的作用。方法 对21只兔采用改良Allen,打击法分别用60、120gcf gram×centimeter×force(gcf)冲量打击兔T10-11脊髓,建立不同程度SCI模型,并在伤前及伤后24 h连续记录兔双侧腓肠肌MEP,用Tarlov’s评分衡量兔下肢运动神经功能。结果 脊髓轻度打击伤时,兔MEP波幅明显降低,但恢复良好;中度打击伤时,MEP波幅的下降更明显,且不能完全恢复;重度打击伤时,MEP波均无法引出,至24 h也不能恢复MEP波形。结论 MEP可敏感而准确地反映脊髓损伤后中枢神经功能的损伤程度,可作为一种有效的监护脊髓功能的手段,并可作为判断运动神经功能预后的可靠指标。
【Abstract】 Purpose To study the relationship between graded spinal cord injury (SCI) and motor evoked potential(MEP), and the role of MEP in the judgement of motor function prognosis. Methods We used modified Allen’ s method by weight drop force of 60,120, 180gcf on the T10 -11 spinal cord of rabbits, and set up graded spinal cord injury models. MEP was recorded continuously at bilateral gastrocnemius muscles before and after the lesion up to 24 hours. The Tarlov’ s technique was used to assess motor neurological function. Results It showed that after spinal cord was mild injured the amplitude of rabbit’ s MEP lowered much obviously, but recovered well. MEP obviously and recovered incomplitely. after spinal cord was moderately injured lowered MEP disappeared and could not recover after spinal cord was severously injured. Conclusions The changes of MEP are related to neurological function. MEP can reflect sensitively and accurat-edly the severity of the central neurological function injury,and can be used as a reliable method in monitoring cord injury and judging neurological prognosis.
