乙型肝炎病毒母婴垂直传播与Th1/Th2型细胞因子相关性研究

【摘要】 目的 :研究乙型肝炎病毒 (HBV)母婴垂直传播与T辅助细胞 1、2型细胞因子的相关性。方法 :将研究对象分为两组 :研究组为 78例乙型肝炎病毒表面抗原 (HBsAg)阳性孕妇 ;对照组为 40例正常孕妇。采用双抗夹心酶联免疫吸附法 (DAS-ELISA)检测孕妇外周静脉血及其新生儿脐静脉血血清中乙型肝炎五项指标 (HBVM)及细胞因子干扰素γ(IFN γ)、白细胞介素 12 (IL 12 )、白细胞介素 6 (IL 6 )水平。结果 :研究组孕妇分娩的新生儿有 10例宫内感染 ,宫内感染率为 12 82 %。胎儿宫内感染组孕妇血清中IFN -γ、IL - 12水平显著低于宫内未感染组及对照组孕妇 ,IL - 6水平则显著高于宫内未感染组及对照组孕妇 (P <0 0 5 )。宫内未感染组与对照组相比 ,上述三种细胞因子水平差异均无显著性(P >0 0 5 )。上述各组孕妇血清中IFN -γ与IL - 12水平均呈显著正相关 (P <0 0 1,P <0 0 5 ,P <0 0 5 ) ;IL - 12与IL -6呈显著负相关 (P <0 0 1,P <0 0 5 ,P <0 0 5 ) ;IFN -γ与IL - 6亦呈显著负相关 (P <0 0 1,P <0 0 5 ,P <0 0 5 )。各组新生儿脐血清中IFN -γ、IL - 12、IL - 6水平差异无显著性 (P >0 0 5 )。结论 :孕妇细胞免疫功能紊乱导致Th1型细胞因子IFN -γ、IL - 12水平下降 ,而Th 2型细胞因子IL - 6水平上升

【Abstract】 Objective: To Study on the relationship between maternal-infant vertical transmission of hepatitis B virus and T helper cells 1, 2 type cytokines. Methods:The pregnant women were divided into two groups: 78 cases with positive HBsAg were included in the study group, the control group included 40 cases of normal pregnant women. By using diantibody sandwich enzyme linked immunosorbant assay(DAS-ELISA), the serum HBV M, IFN-γ, IL-12, and IL-6 levels in maternal and neonates were determined respectively. Results:In the study group, the incidence of HBV intrauterine infection was 12.82%(10/78). The IFN-γ, IL-12 levels of the pregnant women with intrauterine infection were significantly lower than those without intrauterine infection and control group(P<0.05), IL-6 levels was contrary to IFN-γ and IL-12(P<0.05). There had no significan difference of IFN-γ, IL-12, IL-6 levels between women without intrauterine infection and control group(P>0.05). The positive correlation existed between IFN-γ and IL-12 levels among each group(P<0.05). IL-12 and IL-6 levels were negatively correlated, and so were IFN-γ and IL-6 levels(P<0.05). No significant difference of IFN-γ, IL-12, IL-6 levels as found among in newborns group (P> 0.05). Conclusions:There is imbalance of Th1/Th2 cells in pregnant women, which causes IFN-γ and IL-12 activities decrease, and the activity of IL-6 increases. Thus the antiviral responses of IFN-γ and IL-12 are inhibited, resulting higher incidence of intrauterine infection.
