微电极导向立体定向下苍白球毁损术治疗帕金森病(附32例报告)

【摘要】 应用微电极记录与刺激技术 ,在立体定向下对 3 2例帕金森病患者行苍白球内侧部 (GPi)毁损术 ,并在术中对GPi的某些生理学特性进行了研究 ,采用统一帕金森病评定量表 (UPDRS)积分评估手术效果。结果术中靶点更换率 71 9% ,变更范围 2~ 7mm。手术效果优良 ,有效率 10 0 % ,UPDRS平均改善 ( 44 1± 9 5 ) % ,无永久并发症。认为微电极的应用能显著提高苍白球切开术的准确性 ,减少手术并发症 ,提高手术效果

【Abstract】 To introduce the role of microelectrode recording in operation for Parkinson’s disease and enhance the treatment effect,thirty-two patients with Parkinson’s Disease underwent pallidotomy by means of microelectrode recording technology.Some physiological characteristics of interior segment of globus pallidus (Gpi) was studied during the operation.The results shows that,the ratio of changing target location was 71.9%.The modulated distance vary from 2 to 7mm.The mean UPDRS decreased (44.1±9.5)% after operation.No patient shows permanent complication.These suggests that neurophysiological technique can increase the accuracy and safety of pallidotomy and give a better outcome.
