【摘要】 目的 :探讨中晚期青光眼患者眼部血流动力学的改变及手术前后的变化。方法 :采用彩色多普勒超声检测仪测定正常组 3 0例、60眼 ,青光眼组 60例、60眼的眼动脉 (ophthalmicartery,OA)、视网膜中央动脉(centralretinalartery ,CRA)、后短睫状动脉(shortposeriorciliary,SPCA)的血流动力学参数 ,同期观察眼压、视野变化。并检测青光眼手术前后血流参数的变化。结果 :(1 )检测青光眼组与正常组的OA ,CRA ,SPCA ,显示 2组血流动力学差异有高度显著性 ,青光眼血流速度低于正常眼 ,而阻力指数高于正常眼 (P<0 .0 1 ) ;(2 )在青光眼组与正常组检测的眼压差异有高度显著性 (P <0 .0 1 ) ;(3 )青光眼术后CRA的PSV、EDV有上升趋势 ,但阻力指数变化不明显 (P>0 .0 5 )。结论 :眼局部血液循环障碍在中晚期青光眼中起重要作用 ,阻力指数可作为青光眼动力学检测的可靠指标 ,降低血管阻力 ,改善眼局部循环 ,为治疗青光眼开辟了新的途径 ,为手术预后和临床用药提供参考依据。更多还原

【Abstract】 Objective:To discuss the change of blood dynamics of patients with middle and later stage glaucoma and the difference before and after operation. Method:Parameters of blood dynamics of OA?CRA?SPCA in normal group (30 patients,60 eyes) and glaucoma group (60 patients,60 eyes) were mensurated with color Dopple ultrasonic. The change of ophthalmic pressure and feed of vision were observed at the same time, and were compared the parameter of blood flow before and after operation.Result:(1)The velocity of blood flow slowed at the end of diastolic period in OA,CRA,SPCA in both groups( P <0.01);(2)There was significant difference in ophthalmic pressure between two groups( P <0.01);(3)PSV and EPV in CRA had trended to increase,but the chande of drag index was not signifieant ( P >0.05) .Conclusion:The obstruct of local blood flow of ear takes as an important role in glaucoma of middle and later stage. Drag index is a credible index of the check of blood dynamics in glaucoma patients. Lowing the blood vessel resistance and improving the local microcirculation are new ways to treat glaucoma,and are reference for prognosis and drug using.更多还原
