【摘要】 目的　探讨国产奎硫平治疗精神分裂症的疗效及副反应。方法　将 10 0例精神分裂症患者随机分为奎硫平组 (5 0例 )和利培酮组 (5 0例 ) ,进行临床对照试验 ,两组药物治疗剂量分别为 4 0 0～ 6 0 0mg/d和 4～8mg/d ,疗程 8周。疗效指标包括阳性和阴性症状量表 (PANSS)及临床总体印象量表 (CGI) ,不良反应指标为不良反应症状量表 (TESS)及有关实验室检查。结果　治疗结束时 ,两组PANSS评分较入组时均显著减低 (P <0 .0 1)。PANSS总减分率 :奎硫平组为 (5 2 .6± 10 .2 ) % ,利培酮组为 (71.8± 11.6 ) % ;临床总有效率 :奎硫平组 73.9% ,利培酮组 91.8% ,两组间疗效有显著性差异 (P <0 .0 5 )。奎硫平组的不良反应较利培酮组少 ,其中静坐不能和肌强直等EPS发生率显著少于利培酮组 (P <0 .0 1) ,未出现月经失调 (闭经、经期过长或过短 ) ,泌乳、男性乳房发育、性欲减退、勃起障碍等副作用。结论　国产奎硫平治疗精神分裂症是一种有效、安全、经济的药物 ,虽然疗效稍逊于利培酮 ,但不良反应比利培酮轻微 ,尤其适合对锥外反应和催乳素敏感者使用更多还原

【Abstract】 Objective To explore the therapeutic and side effects of domestic quetiapine in the treatment of schizophrenia.Methods Patients who met CCMD 2 R criteria for schizophrenia were randomized into two groups:quetiapine group( n =50) and risperidone group( n =50).The range of dosage for the two drugs were 400～600 mg/d and 4～6mg/d,the treatment duration was 8 weeks.PANSS,CGI,TESS and related lab tests were employed for assessing the therapeutic and side effects.Results At the end of treatment,the scores of PANSS in both treatment groups decreased significantly compared with baseline( P <0.01),and the decreasing rates of PANSS were(52.6±10.2)% in quetiapine group and (71.8±11.6)% in risperidone group. Total clinical effective rates were 73.9% in quetiapine group and 91.8% in risperidone group,and there was significant difference between two groups.However,patients in quetiapine group reported fewer side effects than those in risperidone group,especially in the EPS. In quetiapine group,menstruation disorder (amenorrhea,prolonging or shortening of menstrual period),lactation,male’s mammo development,lack of sexuality and erective dysfunction didn’t appear.Conclusion It is suggested that domestic quetiapine was an effective,safe and cheap drug in treating patients with schizophrenia. It has less side effects,and is suitable to the patients who are sensitive to the EPS and PRL particularly,though it’s not as good as risperidone.更多还原
