兔颅内压增高与大脑中动脉血流速度关系的数学模型

【摘要】 目的:观察兔急性颅内压增高对大脑中动脉血流速度(middle cerebral arterial veocity,V_MCA)的影响,并探讨颅内肿瘤所致急性颅内压(intracranial pressure,ICP)增高的非创伤性检测方法。方法:以硬脑膜外水囊法建立兔急性ICP增高模型,同时以经颅多普勒(transcranial Doppler,TCD)监测V_MCA;以所得数据模拟兔ICP与V_MCA的关系曲线,并进而建立ICP与V_MCA关系的数学模型。结果:ICP与V_MCA关系的方程式为:V=832/[1+exp(1.746 44×P-7.843 31)](其中V为大脑中动脉血流速度,P为颅内压)。结论:动物实验证实,急性ICP增高时V_MCA呈倒“S”形下降。方程较好地模拟了这一实验结果,提示TCD检测V_MCA和数学模型的方法可能成为颅内肿瘤所致急性ICP增高的非创伤性检测手段。

【Abstract】 Objectives To observe the relationship of intracranial hypertension and middle cerebral arterial velocity ( VMCA),and evaluate non-invasive approach to diagnose the acute intracranial hypertension induced by intracranial tumors. Methods The model of acute intracranial hypertension was established by means of bolus infusing into the epidural in rabbits while VMCA detected by TCD, The data were taken advantage to design an equation for analyzing the relationship of intracranial hypertension and VMCA. Results The relationship of intracranial hypertension and VMCA seemed to be a S-shaped curve primarily. The equation of pressure-velocity relationship was V = 832/ [ 1 + exp (1. 746 44×P-7. 843 31) ] ( V was the value of VMCA and P the value of ICP) .Conclusions The mathematical model accurately simulates experimental result of VMCA descend following by increased intracranial pressure. The present simulation strongly indicates that intracranial pressure dynamics can be obtained from VMCA explored by TCD.
