【摘要】 目的 :为了探讨中晚期食管癌的综合治疗方法。方法 :自 1991- 0 4～ 1997- 0 6采取食管渗疗器局部渗疗同时配合 6MVX线三野 (前一后二 )外照射治疗 5 0例中晚期食管癌 ;另 5 0例行单纯三野外照射作为同期对照研究。观察近期疗效及 1、3、5a生存率。结果 :近期疗效 :渗透治疗组与单纯放疗组的CR、PR、CR +PR、NC、PD分别为 5 5 .1%和 10 %。 4 0 .8%和 6 0 % ;95 .92 %和 70 % ;0和 2 0 % ;4 .0 8%和 10 %。CR、CR +RP ,P均为 0 .0 1。生存情况 :1、3、5a生存率为 4 8.1%和 2 7.5 % ,P <0 .0 5 ;5a生存率为 12 %和 0 ,P <0 .0 5 .结论 :食管渗疗器局部渗疗配合 6MVX线外照射治疗中晚期食管癌 ,有显著的近期疗效 ;同时显著的改善了患者的生存 ;其原理可能为明显地提高了癌组织局部的化疗药物浓度 ,最大限度地杀伤癌细胞 ,同时也起到了消除局部炎症及对放疗增敏作用。对食管瘘的修复疗效尤为显著 ;有应用、推广价值更多还原

【Abstract】 Objective:To observe the effect of topicdeffusion chemotherapy combined with radiotherapy in esophageal cancer.Methods:From April 1991 to June 1997,100 cases of esophageal cancer were randomly divided into two groups: The study gruop,(50 cases) received topic diffusion chemotherapy combined with 6MVX externalirradiation, The control group, (50 cases) received 6MVX external irradiation alone. The near effect and the 1?3?5 year survival rates were observed.Results:The near effect: The cR,PR,CR+PR,NC,PD,Were 55.1%,40.8%,95.92%,0,4.08% in study group and 10%,60%,70%,20%,10% in control group (p<0.01). The survival situation: The 1?3?5 year survival rates in study group and control group were 73.46% and 52%(p<0.05),48.15% and 27.5%(p>0.05) 13.51% and 0 (p<0.05).Conclusion:The effect of topic diffusion chemotherapy combined with radiotherapy in esophageal cancer is more superior than the single radiotherapy. The principle of treatment maybe that increase the local cancentration of drugs and radiosensitizing. The effect of repairing for esophageal fistula is better.
