【摘要】 目的　探讨细胞因子IL 10 ,IL 12和INF γ在十二指肠球部溃疡 (DU)发生中的作用。方法　在奥美拉唑、克拉霉素、替硝唑 1周三联疗法治疗之前和之后 4～ 6周从DU患者的球部钳取粘膜组织 ,应用酶联免疫吸附测定 (ELISA)检测IL 10 ,IL 12和IFN γ的含量 ,并与对照组进行比较。结果　 1周三联疗法治疗前 ,DU患者球部粘膜IL 10 ,IL 12和IFN γ含量分别为 17.983± 8.676ng L ,4 4 .0 98± 12 .84 2ng L ,82 .373± 17.2 4 6ng L ,均显著高于对照组 (分别为 8.5 99± 4 .4 17ng L ,17.5 5 0± 5 .980ng L和 2 6.2 98± 10 .385ng L ,P <0 .0 1) ,且IL 10与IL 12的含量呈正相关 (r=0 .87,P <0 .0 1) ,IL 10与IFN γ的含量也呈正相关 (r=0 .92 ,P <0 .0 1)。治疗后 ,DU患者球部黏膜的IL 12 ,IFN γ含量均比治疗前有所下降 (分别为 17.2 89± 3.939ng L和 2 7.0 30± 3.75 3ng L ,P <0 .0 1) ,但IL 10的含量仍持续在较高水平 (2 6.0 98± 15 .15 2ng L ,P <0 .0 1)。结论　细胞因子IL 10 ,IL 12和IFN γ可能在DU的发生机制起重要作用。 1周三联疗法治疗后 ,促炎症细胞因子IL 12和IFN γ的分泌得到有效抑制 ,而抗炎症细胞因子IL 10持续维持在较高水平 ,为粘膜的修复和重建营造了有利环境更多还原

【Abstract】 Objective To investigate the clinical significance of interleukins(IL-10,IL-12) and interferon (IFN-γ) in duodenal mucosa in patients with duodenal ulcer (DU).Methods Doudenal mucosal specimens were taken from patients with Du before and 4～6 weeks after the treatment with omeprazole,clarithromycin and tinidazole for one week.The levels of IL-10,IL-12 and IFN-γ were determined by enzyme-linked immunosorbent assay(ELISA),and compared with the controls.Results Prior to the one-week triple therapy,the levels of IL-10,IL-12 and IFN-γ were markedly increased in patients with DU(17.983±8.676ng/L,44.098±12.842ng/L and 82.373±17.246ng/L)than those in controls (8.599±4.417ng/L,17.550±5.980ng/L and 26.298±10.385ng/L,P<0.01,respectively).A positive relationship between the IL-10 and L-12(r=0.87,P<0.01) was observed,so was the relationship between IL-10 and IFN-γ(r=0.92,P<0.01).After the treatment,the levels of IL-12 and IFN-γ decreased(17.289±3.939ng/L and 27.030±3.753ng/L,P<0.01),but the IL-10 remained at a high level (26.098±15.152ng/L,P<0.01).Conclusion Cytokines IL-10,IL-12 and IFN-γ may play important roles in DU.After the treatment,proinflammatory cytokine IL-12 and IFN-γ were effectively restricted,but in contrast,the contra-inflammatory cytokine IL-10 remained high for a period of time to creat the favorable environment for mucosal reconstruction.更多还原
