【摘要】 目的　分析纤支镜对周围型肺癌的诊断价值。方法　采用纤维支气管镜及病理学方法对 2 2 6例患者进行观察。结果　经纤支镜检查明确诊断 10 6例 ,占总数的 4 6 9%。其中有 9例根据影像定位在盲检及刷检情况下也获得了阳性结果。 2 9例影像检查病灶在外周 ,而纤支镜可见主气管和 (或 )左、右支气管已经受累 ,本组周围型肺癌以腺癌及鳞癌为多见 ,占总数的 6 6 4 % ,其次为小细胞癌 (19% ) ,未定性癌 (9 3% ) ,腺鳞癌 (3 5 % )和大细胞癌 (1 8% ) ,周围型肺癌纤支镜下特征以间接征象为多见 ,主要表现为粘膜的浸润、充血水肿、糜烂、管腔狭窄。有 10 3例 (45 6 % )镜下未见异常。结论　纤支镜对周围型肺癌具有很高的诊断价值 ,周围型肺癌以腺癌和鳞癌多见 ,其镜下特征主要以间接征象为主更多还原

【Abstract】 Objective To study the diagnostic value of bronchoscopy on peripheral lung cancer.Methods The result of fiberbronchoscopy was compared with pathological examination of biopsy specimens in 226 patients with peripheral lung cancer.Results 106 of peripheral lung cancer were proved by fiberbronchoscopy,acounting of 46.9%,9 cases were found in the randorn specimens of mucosa,according to location of tummor under x ray and CT,tracheae and or bronchi impairment were found under fiberbronchoscopy in 29 cases.Squamous cell carcinoma were confirmed in 69 cases(30.5%),adenocarcinoma in 81 patients(35.8%),small cell lung cancer,mixcd squamo adenocarcinoma,unconfirmed and cell type of the lung cancer and large cell carcinoma were confirmed in 43(19%),8(3.5%),21(9.3%) and 4(1.8%) respectively.The indirect characteristics of peripherical cancer under fiberbronchoscopy were more than those of direction.They mainly showed atypical findings of bronchial stenosis,inflammation and edema of bronchial mucous memberane.No abnormal apperances were found under FB in 103 patients.Conclusion Fiberbronchoscopy have high diagnostic value for peripheral lung cancer,the characteristics of which under fiberbronchoscopy mainly showed indirect signs of cancer.更多还原
