猪缓慢进行性大量脑叶出血模型的建立

【摘要】 目的:建立猪脑叶缓慢进行性大量出血模型,适应脑出血病理生理和微创治疗研究的需要。方法:通过注射不抗凝的自体股动脉血2.5ml造成中等量脑出血;通过血肿内注射2.3ml的动脉血后又在血肿内注射肝素500单位的方法造成缓慢进行性出血而形成较大的血肿。通过动态MRI观察、病理、计算组织内干湿重等方法验证模型的稳定性和灶周水肿的范围、程度。结果:T2*WI显示单纯注射2.5ml的不抗凝动脉血后血肿体积在1h、24h内无明显变化(分别为2.29±0.18和2.29±0.18cm~3,P>0.05),而注射肝素后24h时(5.60±0.31cm~3)血肿较1h内(3.51±0.49)明显增大(P<0.001)。24h时在2组T2WI和FLAIR均显示灶周的血脑屏障破坏、明显的血管源性脑水肿。结论:通过血肿腔内注射肝素的方法可建立稳定的进行性大量猪脑叶出血模型。

【Abstract】 Objective: To make a new intracerebral hemorrhage model with enlargement in porcine, which can be used in the field of the phathophysiology about perilesion and its aspiration in the future study. Methods: A medium volume of hematoma was formed by infusing 2. 5ml autologous artery blood. After injecting 500U heparin in the middle of the hematoma, its volume wa gradually enlarged within the first 24 hours, which was identified by MRI and pathology. Results: The T2* WI showed that the hematoma volume at 24hth was similar with that within 1h(2. 29±0. 18cm3 vs 2, 29±0. 18cm3, P>0. 05) after infusing 2. 5ml blood, but after injecting heparin, it was significantly increased at 24h, lardger than that within 1h (5. 60 ± 0. 31cm3 vs 3. 51± 0. 49, P<0. 001). It was clear that the BBB in the perilesion was damaged and significant edema was formed showed by T2WI and FLAIR sequences in animals both with medium and large volume of hemotoma. Conclusion: Injecting heparin in the middle of hematoma results in enlargement of hematoma and a reproducible large volume of hematoma model can be made.
