【摘要】 目的　探讨Bowen样丘疹病的临床病理特征、鉴别诊断及病因。方法　对 15例Bowen样丘疹病进行临床和病理分析并随访。采用多重引物PCR检测 12例Bowen样丘疹病上皮组织中HPVDNA ,用免疫组化S P法检测 15例HPV抗原及p5 3蛋白。 结果　 15例Bowen样丘疹病患者全为女性 ,年龄 2 1～ 4 0岁 (平均 2 9 5岁 )。病变外观为色素性扁平丘疹 ,组织学与Bowen病相似 ,但异型性较小。 12例病变组织HPV6、11、16、18型病毒基因检测均为阴性 ,对照组检测阳性。广谱HPV抗原检测阳性 2例 (2 / 15 ) ,p5 3蛋白阳性 2例 (2 / 15 )。结论　Bowen样丘疹病的诊断应该结合典型临床特征 ,该病属良性病变 ,但伴有重度非典型增生病例可视为癌前病变 ,发病与HPV感染的关系尚待肯定。更多还原

【Abstract】 Purpose To study the clinicopathological features, differential diagnosis and etiologic factors of Bowenoid papulosis(BP). Methods The clinical and pathologic features of 15 cases of BP were analyzed. Multiple PCR was used to detect HPV(6,11,16,18) in 12 cases. Immunohistochemistry staining was applied to determine p53 protein and HPV antigen in 15 cases of BP. Results All 15 cases of BP in the group were women, with aged from 21 to 40 years (mean 29 5 years). Grossly, the disease appeared as multiple flat papules with pigmented, and histologically similar to Bowen’s disease. No positive results were detected for HPV DNA in 12 cases of BP, whereas control cases were positive. HPV antigen were present in 2 cases (2/15) and p53 protein were positive in 2 cases (2/15) of BP. Conclusions Making diagnosis of BP should combine with typical clinical features. BP is a benign disease and the case with severe atypical hyperplasia may be considered as a precancerous lesion. The ralationship between HPV and BP is not certified in this study.更多还原
