【摘要】 目的探讨肱骨近端移位骨折的治疗方法及其影响预后的因素。方法对107例肱骨近端移位骨折进行回顾性分析。并按美国肩肘关节医师学会的肩关节评分系统进行疗效评价。结果与健侧肩关节功能比较,患肩平均恢复幅度为:上举75.03%、内旋72.08%、外旋71.65%,完成7项日常生活动作能力82.27%。疗效依二、三、四部分骨折次序而降低。结论肱骨近端移位骨折的治疗,应根据Neer分型选择不同的治疗方法。手术最好是采用局限软组织切开、用较小的内固定对骨折断端进行有效固定。影响其预后的主要因素是损伤类型、治疗方法及时间、伴发病、年龄、医师经验、并发症等。更多还原

【Abstract】 Objective To investigate the therapeutic methods and factors of affecting prognosis in the displacement of the proximal humeral fractures. Methods From March 1991 to March 2002,107patients with displaced proximal humeral fratures were analyzed retrospectively. The efficacy were evaluated by the rating System developed by American Sociaty of Shoulder and Elbow Surgeons.Results Comparing with the shoulder of normal side,the average range of activity of affecting shoulder was reached to elevation in 75 03%,internal rotation in 72 08%and external rotation in 71 65%,and activities of 7 item were finished in 82 27%. Conclusion The selection of different therapeutic methods are according to the Neer typing of proximal humeral fracture.The operation is best to use localized incision of soft tissues and the fracture needs reliable internal fixation for fracture ends.The main factors of influential prognosis of displaced proximal humeral fractures are injury type,therapeutic method and time,concomitant disease,age,therapeutic experience of doctor and complications and so on.更多还原
