痔切闭术治疗痔的临床应用研究

【摘要】 目的评估痔切闭术治疗III度、IV度痔的疗效。方法在同期住院III度、IV度痔患者中各抽取30例,随机分成切闭组与切扎组。分别按照吻合器切闭术与传统的切扎术术式进行手术治疗,对术后两组疼痛等各项指标进行统计学对比分析。结果切闭组在术后疼痛、出血、伤口愈合时间,尿潴留,住院时间及手术时间各项指标均优于切扎组(P<0.05~0.01)。随访6~18个月,切闭组无肛门狭窄、排便障碍、大便失禁等并发症发生,两组均无复发病例。结论吻合器痔切闭术是一种简单、近期疗效可靠、痛苦轻、恢复快、安全的新术式,可代替传统的切扎术对III度、IV度痔的治疗。远期疗效有待观察。

【Abstract】 Objective To evaluate the clinical effects of excision-closed hemorrhoidectomy in the treatment of degree Ⅲ and Ⅳ hemorrhoids.Methods30cases with degree Ⅲ and30cases with degree Ⅳ hemorrhoids were divided randomly into excision-closed group and excision-ligated group.The excision-closed hemorrhoidectomy with anastomat and traditional excision ligated hremorrhoidectomy were performed respectively.Postoperative pain and multiple index were analyzed statistically between the two groups.Results Excision-closed group was better than that excision-ligated group ( P<0.05~0.01 ) in the postoperative pain ,bleeding ,time of wound healing ,urinary retention ,hospitalization time and operative time.With follow-up6to18months ,no anal stricture ,dysdefecation ,fecal incontinence were found in the excision-closed group and no recurrence was found in both groups.Conclusion The excision-closed hemorrhoidectomy with anastomat is simple ,safe ,effective ,rapid recovery ,minor postoperative pain and it can replace the excision-ligated hemorrhoidectomy.Its long-term efficacy needs to further observation. [
