小剂量前列腺素E1在发绀型先天性心脏病急救中的应用

【摘要】 目的:比较不同剂量前列腺素E1(prostaglandin E1,PGE1)在依赖动脉导管的发绀型先天性心脏病中应用的有效性及并发症。方法:回顾我院新生儿科1999~2002年收治的依赖动脉导管开放的28例发绀型先天性心脏病资料,据急救时PGE1的剂量分为两组,A组15例:小剂量组,即PGE1<0.01μg/(kg·min);B组13例:大剂量组,即PGE1>0.01μg/(kg·min)。比较两组病人用PGE1后血氧饱和度(SaO2)及动脉血氧分压(PaO2)改善的程度、起效时间及并发症。结果:A组在SaO2及PaO2改善的程度、时间与B组相比无显著差异(P>0.05),呼吸暂停、高热、低血压等严重副作用A组较B组少,差异有显著性(P<0.01)。结论:小剂量的PGE1可维持动脉导管的开放,及时改善低氧血症。

【Abstract】 Objective:To compare the efficacy and complications of low dosage prostaglandin E1(PGE1) with high dosage PGE1 in ductus dependent congenital heart disease. Methods:28 newborns with ductus dependent congenital heart disease during 1999~2002 were investigated. According to the dosage of PGE1, the patients divided into low dosage group [Group A, 15 cases, PGE1<0.01 μg/(kg·min)] and high dosage group [Group B, 13 cases, PGE1>0.01 μg/ (kg· min)]. The efficacy of PGE1, time of going to effect, side-effects were analyzed. Correlative factors including SaO2, PaO2 and complications were studied. Results:Two groups were efficacious. The improvement of the SaO2 and PaO2, the time of going to effect was no significant difference in two groups. The complications of apnea, hyperpyrexia, hypotension in group A was less than group B, there was significant difference(P<0.01). Conclusion:Low dosage PGE1 can effectively maintain the patency of the ductus arteriosus in neonates, improve the hypoxia. Low maintenance doses allow long-term treatment without serious complications.
