旁气流通气监测观察双腔支气管导管插管患者呼吸功能变化

【摘要】 目的 应用旁气流通气监测法 (SSS)观察双腔支气管导管插管患者围术期呼吸功能变化。方法 肺叶切除术患者20例行双腔插管 ,监测不同时期通气功能和气道力学的变化 ,计算不同时期的死腔率。结果 双肺转向单肺通气及再恢复双肺通气时期通气指标、气道力学和死腔率在手术中有明显变化。结论 SSS可及时准确反映术中呼吸功能及气道力学的变化 ,有利于发现异常情况 ,以便迅速纠正

【Abstract】 Objective To observe the changes of ventilation and respiratory dynamics in patients undergoing bronchial intubation with double channel catheter and monitored by Side Stream Spirometry(SSS). Methods 20pulmonary lobectomy patients undergoing bronchial intubation with double channel catheter.The changes of respiratory dynamics and ventilation function were measured with SSS monitor,rate of deed space was calculated. Results Compliance?PETCO2significantly decreased and Airway pressure?V1?PaCO2?and rate of deed space significantly increased during one-lung ventilation,to resume two-lung ventilation,Compliance?PETCO2significantly increased and Airway pressure?V1?PaCO2?and rate of deed space significantly deˉcreased,but Compliance?PETCO2decreased and V1?rate of deed space increased as compared with those of two-lung ventilation. Conclusion The changes of respiratory dynamics and ventilation function can be found accurately in time by SSS.
