【摘要】 [目的 ]探讨焦炉逸散物致慢性阻塞性肺疾病 (COPD)的剂量 反应关系和危险特征。 [方法 ]对焦炉工和对照组进行问卷调查、肺功能测定和焦炉逸散物累积接触指数估算。 [结果 ]焦炉工咳痰阳性率 ( 16 2 % )和COPD患病率( 14 5 % )皆明显高于对照组 (分别为 8 0 %和 5 6% ) (P <0 0 5 )。控制偏倚并与对照组或参比组比较 ,发现焦炉工接触焦炉逸散物平均累积剂量 (指数 )为 190 8时 ,其咳痰阳性率OR值为 2 8( 95 %CI:1 0～ 7 9) ;平均累积剂量 (指数 )为 3 8 2时COPD患病率OR值分别为 6 0 ( 95 %CI :2 3～ 15 5 )。吸烟指数≥ 3 0 0患COPD的危险性是吸烟指数 <10 0的 3 6倍。[结论 ]暴露于焦炉逸散物和吸烟与COPD的发生有关 ;且有明显的剂量 反应关系。更多还原

【Abstract】 Objective] To explore the risk characteristic of chronic obstructive pulmonary disease (COPD) and dose response relationship between coke oven emissions and COPD. [Methods] A questionnaire survey,lung function test and assessment of accumulative exposure index were made in coke oven workers and control group. [Results] The positive rate of sputum and prevalence of COPD in coke oven workers were higher than those in control group (16 2% vs. 8 0% and 14 6% vs. 5 6%,respectively) ( P <0 05). After controlling bias and comparing to control or reference groups,the OR of positive rate of sputum was 2 8 (95% CI :1 0～7 9) in the group exposed to 190 8 of average accumulative exposure index of coven emissions,while the OR of COPD prevalence was 6 0 (95% CI :2 3～15 5) at 38 2 of the above average accumulative exposure index. The risk of COPD prevalence in the group with ≥300 smoking index was 3 6 times that in the group with <100 smoking index. [Conclusion] It seems that exposure to coke oven emission and smoking could be related to COPD with dose response relationship.更多还原
