α1-抗胰蛋白酶基因多态性与焦炉工慢性阻塞性肺疾病的关系
Effects of α1-Antitrypsin Genetic Polymorphisms on Chronic Obstructive Pulmonary Diseases Related to Coke Oven Emissions
【摘要】 [目的 ]探讨α1 抗胰蛋白酶 (α1 AT)基因多态性对焦炉逸散物致慢性阻塞性肺疾病 (COPD)的影响。 [方法 ]通过对 45 8名焦炉逸散物接触和非接触工人肺功能测定 ,筛查到 5 2名COPD患者 ,按 1∶1配对进行研究。应用PCR RFLP技术检测α1 AT基因多态性。 [结果 ]病例组见 2例纯合子ZZ基因型 ,同时为SS基因型 ,对照组未见ZZ和SS基因型。MS或SS基因型频率在两组间无显著性差异 ,病例组MZ和ZZ基因型频率分别为 11 5 %和 3 8% ,明显高于对照组( 3 8%和 0 0 % ,P <0 0 5 )。携带MZ或ZZ基因型和焦炉逸散物累积接触指数≥ 3 8 2的个体 ,发生COPD的危险性 ,是携带MM基因型和焦炉逸散物累积接触指数 <3 8 2的个体的 2 4.0 5倍 ( 95 %CI :1 2 9~ 44 8 46)。 [结论 ]α1 AT外显子V的MZ或ZZ基因型对焦炉逸散物致COPD有交互或修饰作用。
【Abstract】 Objective] To explore effects of α 1 antitrypsin genetic polymorphisms on chronic obstructive pulmonary diseases (COPD) related to coke oven emissions. [Methods] A 1∶1 matched case control study was conducted in 52 persons with chronic obstructive pulmonary disease screened by lung function tests in 458 workers exposed to coke oven or not. PCR RFLP methods were used to analyze α 1 antitrypsin genetic polymorphisms. [Results] Two persons with both homozygote ZZ and SS genotypes were found in case group,but none in control group. The frequency of MS or SS genotype was of no significant difference between case group and control group ( P >0 05),while the frequeney of MZ and ZZ genotypes in case group was 11 5% and 3 8%,respectively,which was significantly higher than that in control group ( P <0 05). A 24 5 fold (95% CI :1 29~448 46) higher risk of chronic obstructive pulmonary disease was found among MZ or ZZ genotype persons with a higher cumulative exposure index (≥38 2) as compared to the MM genotype ones with the index lower than 38 2. [Conclusion] It seems that MZ or ZZ genotype in α 1 antitrypsin exon V could have an interaction or modification with coke oven emission in chronic obstructive pulmonary disease.
