【摘要】 目的　探讨玻璃体 (晶状体 )切割联合应用肝素对复杂难治性青光眼的治疗效果。方法　临床选择各种类型复杂难治性青光眼 3 1例。其中 ,陈旧性眼球穿透伤 9例 ,视网膜脱离后并发白内障继发青光眼 7例 ,原发性闭角型青光眼术后复发 9例 ,晶状体囊内摘除后玻璃体虹膜后粘连继发青光眼 6例。采用常规三通道玻切术 ,对瞳孔闭锁虹膜后粘连者同时予以处理。术中灌注液内加入肝素 5 0 u· m l- 1 (0 .4m g· ml- 1 )。结果　术后除 1例出现眼球萎缩 ,1例再发青光眼 ,最终行眼球摘除外 ,其余眼压控制良好 ,并有部分患者视力提高。结论　玻璃体切割对复杂难治性青光眼是一种可行有效的方法 ,术中加入肝素对预防术后玻璃体视网膜增生有重要临床价值更多还原

【Abstract】 Objective To investigate the treating results of vitrectomy (lenstomy) company with heparin for complicated and difficult glaucoma.Methods Clinical selective 31 all varieties type difficult treating glaucoma.The old ocular penetrating injury is 9 patients.The secondry glaucoma source of retinal detachment with complicated cataract is 7 patients.Recurrence glaucoma of postoperative is 9 patients.The posterior synechia of the iris after intracapsular cataract extraction is 6 patients.We adopt standared generally vitrectomy,perform iridodialysis for seclusion of pupil.In operating simultaneity,using 50 u·ml -1 heparin in pouring fluid.Results Except 1 patient occurring ocular shrivel,1 patient recurring glaucoma and they were operated the ocular enucleation.The intraocular pressure of other patients were controled to normal.The visual acuity in some patients was increased in varuous degree.更多还原
