心肌细胞外基质和转化生长因子-β1与原发性高血压左室肥厚的关系
The association of extracellular matrix and TGF-β1 with left ventricular hypertrophy in hypertension
【摘要】 目的 :探讨心肌细胞外基质 (ECM)在原发性高血压 (EH)左室肥厚 (LVH)中的作用及转化生长因子 β1(TGF β1)、血管紧张素Ⅱ (AⅡ )及醛固酮 (ALD)对其影响。方法 :选择EH患者 4 7例 [LVH 2 6例 ,非LVH(NLVH) 2 1例 ],同步检测其血中Ⅰ型前胶原羧基端肽 (PICP)、Ⅲ型前胶原氨基端肽 (PIIINP)、TGF β1、AⅡ及ALD水平 ;采用超声心动图仪测定并计算左室重量指数 (LVMI)。并与对照组 2 6例进行对比分析。结果 :EH组血中PICP、PⅢNP、TGF β1、AⅡ及ALD水平均较对照组显著增高 ,以LVH者为甚 ,差异显著 (P <0 .0 1)。相关分析表明 :PICP、PIIINP分别与TGF β1、AⅡ、ALD呈正相关 ,TGF β1与AⅡ、ALD呈正相关 ,血中各指标与LV MI亦呈正相关。结论 :ECM在EHLVH的诊断中具有重要价值 ,TGF β1、AⅡ及ALD协同作用 ,共同对EHLVH造成影响 ,为临床EH治疗中防治LVH提供理论依据。
【Abstract】 Objective:To study the rule of extracellular matrix (ECM) in diagnosing left ventricular hypertrophy in hypertension (EH), also to study the infleunces of transforming growth factor β 1 (TGF β 1), angiotension Ⅱ(AⅡ), aldosterone (ALD) on ECM.Methods:To detect plasma pre collagen carboxylit end peptide I(PICP), pre collagen amino and peptid Ⅲ (PⅢAP), TGF β 1, AⅡ and ALD?and to calculate left vertvicular mass index (LVMI) in 47 patients with EH and in 26 normal subjects. Results:PICP, PⅢAP, TGF β 1, AⅡ and ALD were much higher than that in normal subjects, especially in LVH patients. The levers of TGF β 1, AⅡ and ALD were corelated with the levels of PICP and PⅢAP.Conclusion:ECM has great value in diagnosing LVH in EH.
