硬膜外阻滞和全身麻醉对Ⅱ型糖尿病患者血液流变学的影响
The influence of epidural and general anesthesia on the hemorheology in type Ⅱ diabetic patients
【摘要】 目的 研究Ⅱ型糖尿病患者和非糖尿病患者在连续硬膜外阻滞和全身麻醉下的血液流变学变化。方法 4 6例患者实施连续硬膜外阻滞或全身麻醉 ,其中 2 4例Ⅱ型糖尿病患者 ,2 2例非糖尿病患者 ,根据麻醉方式的不同随机分为四组 ,比较麻醉前后血液流变学的变化。结果 麻醉后糖尿病患者 :(1)硬膜外阻滞组麻醉全血低切和高切粘度、低切和高切还原粘度较麻醉前降低 ;(2 )全麻组血液流变学指标与麻醉前相比无显著性差异 ;(3)硬膜外阻滞组的全血高切粘度比全麻组低。结论 连续硬膜外麻醉使Ⅱ型糖尿病患者的血液粘滞性降低 ,有益于患者脏器的血流灌注
【Abstract】 Objective To compare the change of hemorheology in type Ⅱ diabetic and non diabetic patients under epidural and general anesthesia.Method Forty six patients under epidural or general anesthesia.Twenty four of that were type Ⅱ diabetic patients and twenty two were non diabetic patients,were randomly divided into four groups according to the type of anesthesia.Blood samples were phlebotomized before anesthesia began and sixty minutes after anesthesia.Results Pre anesthesia,the blood viscosity of the diabetic patients was higher than that of the non diabetic patients.Peri anesthesia In the diabetic patients:(1)there was significant decrease in epidural anesthesia group on whole blood viscosity at high and low shear rate,and reduced viscosity at high and low shear rate sixty minutes after anesthesia began;(2)in the general anesthesia group,the rheologic variables did not change much sixty minutes after anesthesia began compare to pre anesthesia;(3)sixty minutes after anesthesia began the whole blood viscosity at high shear rate of epidural anesthesia group was lower than that of general anesthesia group.Conclusion Epidural anesthesia can decrease the blood viscosity of type Ⅱ diabetic patients,and is helpful to their organic blood perfusion. [
