【摘要】 目的　探讨局部注射32 P胶体和聚合白蛋白 (MAA)治疗中晚期肝癌的可行性。方法　超声引导下瘤内注射MAA和32 P胶体治疗中晚期肝癌 30例 ,并观察其疗效和毒副作用。结果　治疗后所有患者临床症状均减轻 ,AFP水平明显下降 ;平均肿块缩小率为 5 3.2 5 % ,组织学检查显示治疗区内肿瘤组织完全或部分坏死、纤维化。 6个月、1年、2年和 3年生存率分别为 10 0 %、90 %、76.67%和 43.33% ,平均生存期 19.5个月。治疗前后肝肾功能、血常规和免疫功能均无明显变化 ,局部无并发症发生。结论　对于晚期肝癌患者 ,局部注射放射性核素32 P胶体和聚合白蛋白是一种简单、安全和有效的新方法 ,副作用低、适应证广更多还原

【Abstract】 Objective To evaluate the therapeutic effects and side effects of ultrasound-guided intratumoral injection of macroaggregate albumin(MAA)and colloidal 32P for primary hepatic carcinoma(PHC).Methods The ultrasound-guided intratumoral injection therapy of MAA and colloidal 32P was performed in 30 patients with PHC.The effects and side effects were evaluated on the basis of clinical manifestations,histopathological changes,tumor size,serum alpha-fetoprotein(AFP),liver functions,kidney functions,blood routine and immune function before and after the treatment.Results After treatment,clinical symptoms alleviated,serum AFP values decrease remarkably.The tumor size shrinked in all cases and the average shrinkage was 53.25%.Histopathological examination shows complete or partial necrosis and fibrosis of tumor tissue after the treatment.The survival incidences for 0.5,1,2 and 3 years were 100%,90%,76.67% and 43.33%.The average survival period was 19.5 months.No local complication appeared.ECG,liver functions,kidney functions,blood routine and immune functions of the patients before and after treatment remain no obvious difference.Conclusions Internal radiotherapy using macroaggregate albumin and 32P colloidal for treatment of hepatic carcinoma is effective,simple,safe and wide suitable.更多还原
