冠心病患者血浆肾上腺髓质素与内皮素的变化
Changes of plasma Adrenomedullin and endothelin-1 level in patients with coronary heart disease
【摘要】 目的 探讨冠心病患者血浆肾上腺髓质素 (ADM )、内皮素 (ET 1)含量的变化及其与心功能的关系。方法 应用放射免疫法测定 92例冠心病患者和 3 0名正常对照者 (对照组 )血浆ADM和ET 1水平 ,同时用彩色超声心动图测定 40例急性心肌梗塞患者 (AMI组 )的左心室射血分数。结果 冠心病各组血浆ADM水平均高于对照组 (P <0 .0 5 ) ;AMI组血浆ADM显著高于不稳定性心绞痛组 (UA组 )和稳定性心绞痛组 (SA组 ) (P <0 .0 5 ) ,而UA组与SA组比较差异无显著性 (P >0 .0 5 ) ,AMI患者按心功能分组 ,各组间血浆ADM差异有显著性 (P <0 .0 5 )。血浆ADM水平与ET 1呈显著正相关 (r =0 .5 743 ,P <0 .0 1) ;AMI组血浆ADM水平与左心室射血分数呈显著负相关 (r =-0 .615 2 ,P <0 .0 1)。结论 ADM参与了冠心病及心力衰竭的病理生理过程 ,血浆ADM升高可能与ET 1升高有关
【Abstract】 abstract Objective To observe the changes of plasma adrenomedullin (ADM)and Endothelin(ET 1)levels in patients with coronary heart disease.Methods plasma ADM and ET 1 levels were determined by radioimmunoassay in 92 patients with coronary heart disease and 30 healthy control subjects.The left ventricular ejection fraction (LVEF) in 40 patients with acute myocardial infarction was measured by echocardiography.Results The plasma ADM and ET 1 levels in patients with coronary heart disease were significantly higher than those in healthy control, the plasma ADM levels in AMI patients were significantly higher than those in UA or SA patients ( P <0.05) ,there were no significant differences in plasma ADM and ET 1 between UA and SA patients( P >0.05). There were also significant differences among different classes of cardiac performance according to Killip classification in AMI( P <0.05). There was a significant positive correlation between plasma levels of ADM and ET 1 ( r =0.5743, P <0.01). LVEF negatively correlated with plasma ADM levels in the patients with AMI( r =-0.6152, P <0.01).Conclusion ADM plays a certain role in the process of pathophysiology in patients with coronary heart disease and heart failure. The elevated plasma ET 1 may be responsible for the increase of ADM levels in patients with coronary heart disease.
