2型糖尿病听觉诱发电位的研究

【摘要】 目的 通过对 2型糖尿病患者进行脑干听觉诱发电位 (BAEP)的检测 ,早期发现 2型糖尿病患者听觉通路功能的异常并明确其病变部位、受损程度以及相关因素。方法 用NicoletVikingTV型肌电图 /诱发电位仪对 30例 2型糖尿病患者 (DM组 )及年龄和性别与之相匹配的正常人 30例 (对照组 )进行脑干听觉诱发电位检测。应用SPSS统计软件采用t检验、χ2 检验和直线相关分析的方法比较两组间BAEP各项参数和率的差异 ,分析DM组BAEP参数的变化与病程、血糖、血脂、肾功能之间的关系。结果  ( 1)DM组中 18例 ( 6 0 %)有BAEP异常 ,表现为波形缺失、波潜伏期延长及波幅下降。 ( 2 )与对照组比较 ,DM组患者Ⅴ波潜伏期明显延长 (P <0 .0 5 ) ,波幅明显下降 (P <0 .0 5 ) ;Ⅰ~Ⅲ、Ⅲ~Ⅴ、Ⅰ~Ⅴ波间期明显延长 (P <0 .0 5 )。 ( 3)DM组患者BAEP各参数的改变与病程、血脂、肾功能密切相关 (P <0 .0 5 )。结论 BAEP是检测 2型糖尿病患者听觉传导通路功能的敏感指标 ,可较早期地诊断并显示听觉外周和中枢神经传导功能障碍的部位和程度。控制高血糖、高血脂 ,保护肾功能有利于阻止患者的脑干神经功能的异常和病变的发展。

【Abstract】 Objective To evaluate the function of brain stem auditory pathway in patients with type 2 diabetes mellitus,and to establish a possible relationship between the change of brain stem auditory-evoked potentials (BAEP) and varied clinical parameters.Methods BAEP were examined in 30 cases with type 2 diabetes (group DM) and in 30 age-and-sex-matched healthy subjects as a control (group C).The parameters of BAEP were recorded and analyzed by SPSS.Results BAEP abnormalities were found in 18 (60%) diabetic patients, including waveform disappearance, wave latency prolongation and wave amplitude reduction.Compared with control group, we found a significant longer latency (P< 0.05)as well as a remarkable lower amplitude (P<0.05) of waveⅤ.We also found significant elongations in the mean inter-peak latencies of Ⅰ~Ⅲ,Ⅲ~Ⅴ,Ⅰ~Ⅴ(P<0.05).Furthermore,a positive correlation was found between the parameters of BAEP and the disease duration,the renal function,the level of blood lipid(TG and CH) separately (P<0.05).Conclusions The pathological changes of peripheral and central nervous system in type 2 diabetic patients,and BAEP technique was a sensitive and helpful indicator of early diagnosis of diabetic neuropathy.Lowering high blood glucose and lipid and protecting the renal function would be beneficial to prevent the impairment of auditory pathway in diabetics.
