【摘要】 目的 研究咽后壁瓣术后不同时期瓣外观形态的改变及对呼吸道阻塞等并发症的影响。方法将10只健康杂种犬随机分为两组,每组5只,分别作上、下蒂型咽后壁瓣手术,观察其临床症状。每组再随机分为5亚组,分别于术后即刻、3、7、30、90天处死,将瓣取下后观察其外观形态的变化。结果上蒂型咽后壁瓣术后早期瘀血水肿,上呼吸道阻塞,鼾声明显,以术后2-3天时最严重,以后逐渐改善,2周后可消失,远期瓣体未见明显萎缩。下蒂型咽后壁瓣术后早期轻度水肿,上呼吸道阻塞不明显,瓣尖至瓣中部黏膜缺血坏死,逐渐为口腔黏膜取代,远期瓣体未见明显萎缩。结论上蒂型咽后壁瓣术后早期瓣瘀血水肿,下蒂型咽后壁瓣术后早期瓣尖缺血坏死。上、下蒂型咽后壁瓣远期都无明显萎缩。更多还原

【Abstract】 Objectives To investigate the contour changes and the complications of the posterior pharyngeal flap in different postoperative periods. Methods Ten mongrel dogs were randomly divided into two experimental groups, which were performed superioly and inferiorly based posterior pharyngeal flap surgery respectively. Each group was then subdivided into 5 subgroups, and were sacrificed immediately after operation and on 3d,7d,30d,90d postoperative respectively. The symptoms were observed and the contour of the flaps were measured after the flaps have been taken down. Results (1) The congestion and edema were found in superiorly based posterior pharyngeal flap in the early postoperative periord. The edema was most severe in the 3d flap. The upper airway obstruction was most obviously in the 2 ?3th postoperative day and was relieved gradually in 14 days postoperative. (2) The mucous membrane of the distal and middle part of the inferiorly based posterior pharyngeal flap was ischemic and necrotic in the 3d and 7d flap and reepithelized in the .itld and 90d flap. Slight edema and upper airway obstruction were found temporaly in the early postoperative period in the inferiorly based posterior pharyngeal flap. (3) No atrophy and dehiscence were found in all flaps. Conclusions The congestion and edema of the superiorly based flap cause the upper airway obstruction in the early postoperative period. Both superiorly and inferiorly based flaps can maintain normal contour.更多还原
