多层面螺旋CT Pinpoint的临床应用
Clinical Application of Pinpoint Stereoscopic System in Multislice Helical CT-guided Biopsy
【摘要】 目的 探讨立体定向穿刺系统 (Pinpoint系统 )在多层面螺旋CT(MSCT)的临床应用价值。资料与方法 对 32例发现体部肿块者进行MSCT下Pinpoint系统导向穿刺。包括肺部病变 2 5例 ,纵隔 4例 ,胸腔入口、肝、胰各 1例。取出标本分别作组织学或细胞学诊断。评价穿刺的到位率、病检的准确性和并发症的发生率。结果 32例经Pinpoint系统均成功取材 ,穿刺针到位率 10 0 % ,一针刺中率 71.9%。穿刺病理诊断准确性 87.5 % (2 8/32 )。 5例 (15 .6 % )发生并发症 ,其中 1例出血 ,4例气胸。结论 Pinpoint系统与MSCT配合使用可以取得较高的活检准确率 ,是一种准确、安全的定位方法
【Abstract】 Objective To evaluate pinpoint stereoscopic system in multislice CT (MSCT) guided biopsy. Materials and Methods Directed by pinpoint stereoscopic system, MSCT-guided puncture biopsy of mass in the body was performed in 32 patients. The punctured organs included lung (n=25), mediastinum (n=4), thoracic inlet (n=1), liver (n=1) and pancreas (n=1). The specimens were sent for cytologic or histologic exam. The puncturing success rate, the biopsy diagnostic accuracy and the complications were evaluated.Results The sample-catching was successful in all 32 patients. The accuracy of puncturing site was 100%, with single puncturing success of 71.9%. The pathologic diagnosis showed an accuracy of 87.5% (28/32). Complications occurred in 5 cases (15.6%), including bleeding (n=1) and pneumothorax (n=4). Conclusion Directed by pinpoint stereoscopic system, MSCT-guided puncture biopsy accuracy can be considerably improved. This technique is a safe and valuable orientation method.
