【摘要】 目的 :探讨种植义齿龈沟液IL-6浓度同种植义齿周围组织状态的关系 ,评价其作为种植义齿周围炎诊断的客观指标的意义。方法 :将86颗种植义齿按临床及影象学标准分为健康种植义齿 (HI)、种植义齿粘膜炎 (MI)和种植义齿周围炎 (PI)三组 ,滤纸条法收集龈沟液 ,双抗体夹心ELISA法进行龈沟液IL-6水平的检测。结果 :(1)三组种植义齿龈沟液IL -6浓度分别是HI为1.74±0.66ng/ml,MI为3.53±1.57ng/ml,PI为10.36±6.45ng/ml。(2)三组的IL-6浓度有非常显著性差异 (P<0.01) ,其中PI的IL-6浓度大于HI和MI有非常显著性差异 (P<0.01),HI和MI之间无显著性差异 (P>0.05)。 (3)IL-6浓度水平同各临床指标都呈显著正相关 ,同PLI为 (r=0.57;P<0.01),同mBI为 (r=0.67;P<0.01),同PD为 (r=0.54;P<0.01),同BL为 (r=0.64;P<0.01)。 (4)当以IL-6浓度大于6.0ng/ml作为诊断种植义齿周围炎的标准时 ,诊断实验的敏感度=79 % ,特异度=72% ,阳性预测值=63% ,阴性预测值=76%。结论 :龈沟液中IL-6浓度的测量可作为诊断种植义齿周围炎的一个有效手段。更多还原

【Abstract】 Objective:The aim of this study was to determine the relationship between the level of IL-6in peri- implant crevicular fluid(PCF)and peri-implant condition in order to assess if IL-6in PCF can be further stud-ied as a possible objective diagnostic aid in oral implantology.Method:86fixtures were divided into3groups,43healthy implants(H).31implants with mucositis(MI)and12implants affected by peri-implants(PI)accord-ing to well-defined clinical and radiographic criteria.PCF was collected by paper strip and was detected by using ELISA method.Results:The level of IL-6in HI was1.74±0.66ng/ml;in MI was3.53±1.57ng/ml;in PI was10.36±6.45ng/ml.The defference among HI,MI and PI was statistically significant at P<0.01level,A significant difference existed between HI/P and MI/PI,No statistically significant difference was found between HI/MI.The level ofIL-6inthe PCFwas significantly(P<0.01)associatedwithplaque index(r=0.57),mod-ificationsulcus bleedingindex(r=0.67),probingdepth(r=0.54)and bone loss(r=0.64).Whenthe thresh-old for a positive IL-6test was set≥3.0ng/ml,a sensitivity=0.79and a specificity=0.72were found in the detection of peri-implants,The positive predictive value was63%and negative predictive value was76% . Conclusion:Measurement of IL-6in PCF may be an important supplement to clinical findings in establishing a diagnosis of peri-implantitis.更多还原
