咬合重建修复过程中咀嚼肌肌电图的变化

【摘要】 目的:探讨对垂直距离降低的患者咬合重建修复过程中,咀嚼肌肌电图的变化。方法:对15例垂直距离降低的患者先用塑料垫式可摘局部义齿过渡修复并恢复垂直距离,患者适应后再按此高度永久修复。对比分析修复前、过渡修复一个月后以及永久修复一个月后嚼肌、颞肌前束和颞肌后束表面肌电图的变化。结果:① 息止位时过渡修复及永久修复后嚼肌、颞肌前束和颞肌后束肌电幅值较术前均有明显减少趋势;② 大力咬合位,过渡修复时颞肌后束肌电幅值较术前显著减少,嚼肌和颞肌前束肌电无显著改变。永久修复后嚼肌和颞肌前束肌电较过渡修复时和术前有显著提高,颞肌后束肌电值较过渡修复时也显著增加;③ 咀嚼时嚼肌和颞肌前束肌电值在过渡修复及永久修复后逐步增加,永久修复后增加显著。而颞肌后束肌电值无明显改变。结论:对垂直距离降低的病人升高垂直距离并进行咬合重建修复后,能显著减少息止位嚼肌、颞肌前束和颞肌后束的紧张度,增强咀嚼肌大力咬合及咀嚼运动时的收缩能力。

【Abstract】 Objective: To investigate the changes in EMG activity of masticatory muscles during occlusal rehabilitationin patients with decreased vertical dimensions. Methods: 15 patients with decreased vertical dimensions were restored byremovable acrylic occlusal splints at increased vertical dimensions first, then permanent occlusal rehabilitation wereconducted. The electromyographic(EMG) activity of masseter and anterior and posterior temporal muscles were comparedamong the stages of pre-treatment、1 month after temporarily restored and 1 month after permanently restored. Results:After temporary and permanent restoration, the EMG amplitudes of masseter and anterior and posterior temporal musclesat rest position decreased. During maximum clenching, a significant reduction was only seen in the EMG of posteriortemporal muscle at the stage of temporary restoration. After permanent rehabilitation, the EMG of these three musclesincreased obviously. While during chewing, the EMG activity of both the masseter and anterior temporal muscles increasedat the stage of temporary restoration, but the difference was not significant. After permanently restored, these two muscleshad significantly increased EMG activities than that of two previous stages. Conclusions: Raising the vertical dimensionand undertaking occlusal rehabilitation can decrease the EMG activity of masticatory muscles at rest position, and increasethe contract strength of these muscles during maximal clenching and chewing.
