【摘要】 目的:观察头孢吡肟治疗慢性阻塞性肺疾病(慢阻肺)下呼吸道感染的临床疗效及安全性。方法:将65例慢阻肺伴下呼吸道感染患者随机分为两组,在常规治疗的基础上,治疗组32例,采用头孢吡肟2g,静脉滴注,每12 h1次;对照组33例,给予头孢他啶2g,静脉滴注,每12h1次,两组疗程均为7～14d。结果:头孢吡肟组治愈率为68.8％,有效率为90.6％,细菌清除率为91.2％;头孢他啶组治愈率为60.6％,有效率为87.9％,细菌清除率为90.3％,两组比较差异无显著性(P>0.05)。两组患者共分离出病原菌65株,药敏试验显示对头孢吡肟敏感率为89.2％,明显高于头孢他啶(78.5％)和头孢噻肟(7.4％,P<0.05)。结论:头孢哟肟的体外抗菌活性强于头孢他啶和头孢噻肟,其治疗慢阻肺伴下呼吸道感染疗效确切、使用安全。更多还原

【Abstract】 Objective: To observe the efficacy and safety of cefepime in treatment of chronic obstructive pulmonary disease (COPD) patients with lower respiratory tract infections. Methods: 65 patients were randomized into two groups. Patients were treated with cefepime 2 g, iv gtt, q 12 h, for 7- 14 d in therapeutic group (n = 32), and ceftazidime 2 g, iv gtt q 12 h in control group(n = 33). Results: In cefepime group, the cure rate was 68. 6% , the effective rate 90. 6% , the bacterial eradication rate 91. 2% . In ceftazidime group, the cure rate was 60.6% , the effective rate 87. 9% ,the bacterial eradication rate 90.3% , there was no statistically significant difference between the two groups. The susceptibility rates of bacteria to cefepime (89.2%) were higher than those of cef-tazidine (78.5%, P<0.05) and cefotaxime (75.4%, P<0.05). Conclusions: The antibacterial activity of cefepime is stronger than that of ceftzidime. Cefepime is effective and safe in the treatment of lower respiratory tract infection patients with COPD.更多还原
